超声在肿瘤性肠梗阻的定性及定位诊断中的价值分析

摘　　要：为探讨超声在肿瘤性肠梗阻的定性及定位诊断中的价值,选取2016年2月至2018年2月我院治疗疑似肿瘤性肠梗阻患者48例,采取超声检查,将手术病理结果作为金标准,比较超声与术后病理结果的定性率与定位率。结果显示,超声检查肿瘤性肠梗阻的敏感度为95.00%(38/40)、特异度为25.00%(2/8)、阳性预测值为86.36%(38/44)、阴性预测值为50.00%(2/4)、准确率为83.33%,2组方式诊断肿瘤性肠梗阻差异无统计学意义(P>0.05)。定位诊断肠梗阻部位29例,其中小肠肿瘤13例,大肠肿瘤16例,定位准确率为60.42%(29/48)。结果表明,肿瘤性肠梗阻采用超声检查,可尽早有效且无创地确定肿瘤性肠梗阻疾病,且超声检查具备的定性与定位诊断具有临床价值。This study was to explore the value of ultrasound in the quality and niveau diagnosis of neoplastic intestinal obstruction( NIO),enrolled 48 suspected NIO patients treated in author's hospital (2016-02-2018-02),which all the patients received ultrasound inspection,and compared the qualitative ratio and the ratio of determined site between ultrasound inspected results and operative pathological results(as golden standard).As results,in ultrasound inspection for NIO sensitivity was 95.00%(38/40),specificity was 25.00%(2/8),positive predictive value was 86.3 %(38/44),negative predictive value was 50.00%(2/4),accuracy was 83.33%;there was no statistical difference between both methods for diagnosing NIO ( P > 0.05);29 cases were of determined intestinal obstruction sites including small intestinal neoplasms of 13 cases, and large intestinal neoplasms of 16 cases, accuracy was up to 60.42 %(29/48).Results show that ultrasound inspection for NIO can as early as possibly and non-invasively determine NIO. and the quality and niveau diagnosis of NIO through ultrasound inspection has clinical value.
