烧伤患者出院半年内希望水平状况及其影响因素

摘　　要：目的了解烧伤患者出院半年内希望水平状况,并探讨其影响因素。方法 2016年9月至2017年5月,便利抽样法选取在福建省某烧伤中心住院患者207例为研究对象。采用中文版希望因子量表(herth hope index,HHI)、改良Barthel指数评定量表(modified Barthel index,MBI)、五级瘙痒评分法、视觉模拟评分法(visual analogue scale,VAS)和烧伤患者一般情况调查表在出院时(T1)、出院1个月(T2)、出院3个月(T3)、出院半年(T4)四个时间点对烧伤患者进行问卷调查。结果烧伤患者T1～T4的希望水平分别为(31.24±7.84)、(31.53±7.53)、(32.71±7.49)、(33.34±7.58)分,呈上升趋势。单因素分析显示,不同职业、经济状况、家庭居住地、烧伤严重程度、烧伤原因、烧伤深度、烧伤后遗症、吸入性损伤、烧伤并发症、瘙痒程度、疼痛程度、康复信心、日常生活能力水平(MBI分值)、工作状况的患者,其T4希望水平不同,且差异有统计学意义(均P<0.05)。回归分析结果表明,经济与工作状况、日常活动能力水平、瘙痒程度、康复信心是烧伤患者出院半年(T4)希望水平的主要影响因素(P<0.05)。结论烧伤患者出院半年内希望水平有待提高,医护人员应以烧伤患者希望水平的影响因素为切入点,在适当时间点实施行之有效的干预,提升其希望水平。Objective To explore hope level in patients with burns and its related factors. Methods From September 2016 to May 2017, hospitalized patients in a burn center in Fujian Province were selected as the research objects by convenience sampling method.Burn patients were investigated by Herth hope index(HHI),modified Barthel index(MBI) scale,5-level itch scale,visual analogue scale(VAS) and patient’s demographic data, and the data was conducted at the time of discharge(T1),one month(T2),three months(T3) and six months(T4). Results The HHI score of burn patients at discharge was 31.24±7.84,the score of 1-month was 31.53±7.53,the score of 3-month was 32.71±7.49,the score of 6-month was 33.34±7.58. The average score of HHI at four points showed upward trend.Univariate analysis showed that the level of hope at T4 was varied in patients with different occupations,economic status, family residence, the severity of burns,the causes of burns, the depth of burns,the sequelae of burns, inhalation injury, burn complications,the degree of itchiness,the level of pain,the confidence of rehabilitation, the ability of daily living(MBI score) and working status, and the difference was statistically significant(P<0.05).Multivariable linear regression showed financial status and working condition,the ADL level,the itching level and rehabilitation confidence were the main influencing factors of hope level of patients with burns at T4(P<0.05).Conclusions The hope level of patients with burns at four time points should be improved.Medical workers should use main influencing factors as point of intervention,and provide effective rehabilitation nursing project at the appropriate time to improve the hope level of burn patients.
