摘　　要：目的:调查阿尔茨海默病患者家庭照顾者的连带病耻感水平并分析其与照顾者负担之间的关系。方法:采用方便抽样法选取符合纳入和排除标准的141例家庭照顾者为研究对象进行问卷调查。结果:阿尔茨海默病患者家庭照顾者的连带病耻感总分为(48.30±9.54)分;单因素分析结果显示不同痴呆严重程度、照顾者的年龄、与患者的关系、照顾时长下,家庭照顾者连带病耻感得分差异有统计学意义(均P<0.05);相关分析结果显示连带病耻感与照顾者负担呈正相关(P<0.01);分层回归分析显示在控制基本资料后,连带病耻感能独立解释照顾者负担21.0%的变异。结论:阿尔茨海默病患者家庭照顾者存在病耻感,并且可预测照顾者负担水平,提示医护人员可针对性地给予心理支持和干预,降低照顾者的连带病耻感水平,改善其心理健康状况,从而减轻照顾者负担。Objectives: To determine the shame among family caregivers of patients with Alzheimer’s Disease and its relationship with caregiver burden. Methods: The convenience sampling method was used to select 141 patients and their caregivers who were investigated by the questionnaires distributed to them. Results: The shame score of family caregivers of patients with Alzheimer’s Disease was(48.30±9.54). According to one-way ANOVA analysis, significant differences were observed regarding to comparison of the severity of Alzheimer’s Disease, caregivers’ age, the caregiver-patient relationship and time of care(P<0.05). In line with correlation analysis, disease-related shame was strongly correlated with caregiver burden. Disease-related shame was analyzed through hierarchical regression analysis, which suggested disease-related shame could account for 21.0% of the variance in dependently after general information was controlled. Conclusion: It was indicated that family caregivers of patients with Alzheimer’s Disease maintained a relatively medium to high sense of shame which was strongly correlated with caregiver burden. Thus targeted and customized psychological support and interventions should be offered to caregivers of patients with Alzheimer’s Disease by medical staff in order to reduce caregiver’s sense of shame and improve their mental health, which can reduce their burden as caregivers.
