摘  要:目的探讨微细血管成像模式背景下超声造影对甲状腺良性结节血流的评估,及其在热消融中的应用价值。方法选取经我院穿刺病理结果证实为结节性甲状腺肿的单发实性或囊实性结节(实性成分≥50%)的患者50例,分别采用彩色多普勒(CDFI)、微细血管成像(SMI)和超声造影(CEUS)及微细血管成像模式背景下超声造影(SMI+CEUS)对结节的血流信号进行分级评估,计算结节的血流信号显示率,并进行统计学分析。结果 SMI+CEUS对结节Ⅲ级血流信号显示率均高于单纯使用CEUS、SMI、CDFI[78.00%(39/50)、40.00%(20/50)、38.00%(19/50)、6.00%(3/50)],SMI+CEUS与单纯使用CEUS、SMI差异有统计学意义(P<0.05),单纯使用SMI与单纯使用CEUS差异无统计学意义(P>0.05),二者间一致性强(Kappa=0.828,P<0.05)。结论 SMI+CEUS对甲状腺结节血流的评估优于单纯CEUS,可更好的指导临床进行甲状腺结节热消融。Objective To evaluate the application of contrast-enhanced in the context of superb microvascular imaging on the blood flow assessment of benign thyroid nodules and its application value in thermal ablation.Methods50 patients with single solid or predominately solid nodule(solid components≥50%)participated in the project,all of which were confirmed to be benign by the pathology of our hospital.All nodules blood flow signal were evaluated by color doppler flow imaging(CDFI),superb microvascular imaging(SMI)and contrast-enhanced ultrasound(CEUS)and CEUS in the context of SMI mode(SMI+CEUS).Grading the blood flow signal of the nodule,calculating the blood flow signal display rate,and performing statistical analysis.Results SMI+CEUS showed higher rates of nodule gradeⅢblood flow signals than CEUS,SMI,and CDFI alone[78.00%(39/50),40.00%(20/50),38.00%(19/50),6.00%(3/50)],There was significant difference between SMI+CEUS and CEUS alone,and SMI alone(P<0.05).There was no significant difference between SMI and CEUS alone(P>0.05).The consistency between the two was strong(Kappa=0.828,P<0.05).Conclusions SMI+CEUS is superior to CEUS in evaluating thyroid nodule blood flow,which can better guide clinical thermal ablation of thyroid nodules.
