摘　　要：目的探讨肺超声评分(LUS)评估儿童重症肺炎及判断预后的价值。方法收集35例重症肺炎患儿。所有受试者行肺部超声检查,并计算LUS;分析LUS与呼吸频率、氧饱和度、住院天数、是否入住重症监护室(PICU)等的相关性;应用ROC曲线评价LUS预测重症肺炎预后的价值。结果重症肺炎患儿LUS与呼吸频率、住院天数呈正相关,与氧饱和度呈负相关(P均<0.05)。以LUS总分20.5分预测患儿需入住PICU的灵敏度与特异度分别为81.3%和63.2%。结论 LUS可以定量评估重症肺炎肺部病变严重程度,并可以预测患儿预后,对临床诊疗具有重要价值。Objective To evaluate the value of lung ultrasound score(LUS)in diagnosis and prognosis of children with severe pneumonia.Methods 35 children with severe pneumonia were studied.Lung ultrasound was performed in all the subjects,and LUS was calculated.The correlation between LUS and respiratory frequency,oxygen saturation,days of hospitalization,and admission to intensive care unit(PICU)was analyzed.ROC curve was used to calculate sensitivity and specificity of LUS to predict the prognosis of severe pneumonia.Results The LUS correlated with respiratory frequency,days of hospitalization,oxygen saturation(P<0.05).The cutoff point for LUS to predict the need of admission to PICU was 20.5 with sensitivity of 81.3% and specificity of 63.2%.Conclusions LUS can quantitative the severity of severe pneumonia in children and predict the prognosis.
