摘 要:目的应用二维超声测量肺动脉内径及McGoon指数评价法洛四联症(TOF)对胎儿肺发育的影响。方法以41例TOF胎儿为研究对象,110例正常胎儿为对照组,二维超声测量肺动脉内径及McGoon指数,三维超声测量肺体积(FLV),对比分析TOF胎儿肺动脉变化,并将肺动脉参数与FLV进行相关回归分析。结果 (1)TOF胎儿肺动脉McGoon指数较正常胎儿明显减低(P<0.05)。(2)矫正胎儿GA、EFW、CTR、DA影响因素后,左右肺动脉内径之和(RPA+LPA)与FLV呈显著正相关关系(P<0.05)。结论 TOF胎儿McGoon指数明显低于正常胎儿,提示此类先心病在胎儿时期可能已经对肺动脉发育产生一定不良影响。RPA+LPA与胎儿FLV呈明显线性正相关,二维超声测量左右肺动脉一定程度上可反映胎儿肺发育情况。Objective The objective of this study was to quantify fetal pulmonary vascular parameters in TOF cases.Methods 41 fetuses with TOF and 110 healthy fetuses were enrolled in the study.Diameters of right pulmonary artery and left pulmonary artery(RPA+LPA)and MGI were estimated by 2 D-ultrasound.Fetal lung volume(FLV)was estimated using 3 D-ultrasound.Results McGoon in TOF group was significantly reduced than in control group,and RPA+LPA was associated with FLV both in the TOF and control groups(all P<0.05).Conclusions MGI values in fetuses with TOF tended to be decreased.We concluded that TOF might delay fetal pulmonary vascular development.FLV was associated with RPA+LPA,and RPA+LPA could be used as an alternative to FLV for assessing fetal lung development.
