赤芍承气汤高位保留灌肠对肝衰竭患者肠道微生态失衡的影响

摘　　要：目的:观察赤芍承气汤高位保留灌肠对肝衰竭患者肠道菌群、血清内毒素水平的影响。方法:60例患者,随机分为2组,各30例。两组患者均采用保肝退黄支持等常规综合治疗,治疗组患者加用承气汤150ml高位保留灌肠,对照组患者采用乳果糖100ml保留灌肠。均2次/wk,连续治疗8周。观察两组患者治疗前后肝功能、凝血指标变化、肠道优势菌群定量、真菌感染及体力、食欲、腹胀、皮肤瘙痒等情况。结果:治疗后治疗组患者肝功能、凝血功能较对照组明显改善,其肠道有益菌亦较对照组明显增加,真菌感染明显减少。结论:赤芍承气汤高位保留灌肠可改善肝衰竭患者肝功能、凝血功能,增加肠道有益菌的生长,减少真菌感染。Objective:To observe the effect of high retention enema with Chishao Chengqi Decoction on intestinal flora and blood endotoxin levels in patients with liver failure.Methods:Sixty patients were randomly divided into two groups,30 in each group.Two groups of patients were treated with routine comprehensive treatment,such as liver-protecting and yellow-eliminating support.The treatment group was treated with high retention enema of 150 ml Chengqi Decoction,while the control group was treated with 100 ml lactulose retention enema.They were treated twice a week for 8 weeks.The changes of liver function,hemagglutination index,quantification of dominant intestinal flora,fungal infection,physical strength,appetite,abdominal distension and pruritus were observed before and after treatment.Results:After treatment,the liver function and coagulation function of the patients in the treatment group were significantly improved compared with those in the control group,and the beneficial bacteria in the intestine of the patients in the treatment group were also significantly increased compared with those in the control group,and the fungal infection was significantly reduced.Conclusion:High retention enema with Chishao Chengqi Decoction can improve liver function and coagulation function,increase the growth of intestinal beneficial bacteria and reduce fungal infection in patients with liver failure.
