基于数据挖掘的吴良村教授辨治肝细胞癌用药规律

摘　　要：目的:分析吴良村教授辨治肝细胞癌用药规律。方法:纳入吴良村教授门诊肝细胞癌87例286诊次,采集临床信息,建立方剂数据库,对药物频数、类别、性味采用描述性分析,对频数最高的48种药物进行相关性分析和系统聚类。结果:共涉及用药189种,累计用药频数5033次,频数最高者为枸杞子,其次为北沙参、白芍、甘草等;类别频数最高者为补虚药,其次为清热药、利水渗湿药、理气药等;性味以甘、寒、苦、平为主;聚类分析将药物分为13类,涉及方剂一贯煎、茵陈蒿汤等,药对知母与生地黄,金钱草与三叶青,龙葵与车前子等,以及相关性分析的药对青蒿与黄芩,黄连与地榆炭。结论:吴良村教授辨治肝细胞癌以益气养阴清热治法为主,辅以解毒散结、健脾渗湿、疏肝和胃、理气化痰等治法,用药善于甘、寒、苦、平配伍。Objective:To analyze the medication rule of WU Liang-cun in differentiating and treating hepatocellular carcinoma(HCC).Methods:Total of 87 patients with HCC visited Wu’s clinic,with 286 floe rates were included.Clinical information was collected and clinical prescription was established to conduct descriptive analysis on the cumulative frequency,categories and tastes of drugs,and the 48 drugs with the highest frequency were analyzed by cluster analysis and correlation analysis.Results:There were 189 drugs were prescribed.The cumulative frequency of medicinal was 5033 in which the fruit of Chinese wolfberry had the highest frequency,followed by Radix Glehniae,White Peony Root,liquorice,etc.The most frequently used medicinal category was tonifing deficiency medicine,followed by the categories of clearing the heat,clearing damp and promoting diuresis,regulating Qi.The natures and tastes of drugs based on sweet,cold,bitter and neutral.The medicines were classified into 13 categories by cluster analysis,involving formulas including Yiguan Jian,Yinchenhao Tang,and medicinal pairs including radix rehmanniae and common anemarrhena,tetrastigma hemsleyanum diels and lysimachia christinae hance,nightshade and semen plantaginis,etc.Artemisia and scutellaria,Coptis and Sanguisorba carbon were medicinal pairs on correlation analysis.Conclusion:WU Liang-cun treats HCC mainly with therapeutic method of enriching qi,nourishing yin and clearing heat,which should be assisted by other therapeutic methods of removing toxin and precluding knot,invigorating the spleen to transform dampness,smoothing the liver and regulating the stomach,regulating qi and melting phlegm.He is good at using medicine compatibilities as sweet,cold,bitter and neutral.
