摘 要:目的了解宁夏地区急性腹泻患者中诺如病毒的流行特征。方法 2016-2017年在宁夏5个市10家哨点医院门诊收集符合纳入条件的腹泻标本2 334例,采用实时荧光定量RT-PCR方法进行定性检测,采用RT-PCR分别对GⅡ型阳性No V标本的Rd Rp区和VP1区进行扩增、测序,并进行基因分型及进化树构建。结果 2 334例粪便标本中(男性1 343例,女性991例),诺如病毒总阳性率为15.68%(366/2 334),GⅠ型阳性率0.86%(20/2 334),GⅡ型阳性率为14.82%(346/2 334);10~20岁年龄组阳性率最高,为27.19%(31/114),其次为0~2岁年龄组,为18.12%(251/1385),50~60岁年龄组阳性率最低,为5.48%(4/73);冬春季发病率高。序列分析显示,GⅡ型No V的Rd Rp区序列分为5种亚型,依次为GⅡ.Pe、GⅡ.P12、GⅡ.P16、GⅡ.P17和GⅡ.P7;VP1区序列分为6种亚型,依次为GⅡ.4 Sydney_2012、GⅡ.3、GⅡ.2、GⅡ.1、GⅡ.17和GⅡ.6。结论 2016-2017年宁夏地区GⅡ型No V存在多种基因型感染,Rd Rp区以GⅡ.Pe和GⅡ.P12为主,VP1区以GⅡ.4 Sydney_2012、GⅡ.3为主。Objective To explore the epidemiologic characteristics of norovirus isolated in outpatients with acute diarrhea in Ningxia between 2016 and 2017.Methods 2334 stool specimens were collected with acute diarrhea admitted in ten sentinel hospitals in Ningxia from 2016 to 2017.Real-time RT-PCR was used for qualitative detection,RT-PCR was used to amplify RdRp and the VP1 region of GⅡ positive specimens,all the positive products were sequenced,and the obtained sequences were analyzed of genotyping and phylogenetic tree.Results There were 2334 specimens collected from 1343 males and 991 females,the total positive rate was 15.68%(366/2334),the positive rate of NoV GⅠ was 0.86%(20/2334),while GⅡ was with a positive rate of 14.82%(346/2334).There was no significant difference in the positive rate of the different genders(P>0.05).However,the highest positive rate was 10-20 age group with 27.19%(31/114),then 0-2 age group with 18.12%(251/1384),50-60 age group was lowest with 5.48%(4/73),and the differences were statistically significant(P<0.05).The positive rates were high in winter and spring,and then increased obviously in 5-7 months in 2017.Additionally,results from the NoV GⅡ sequence analysis demonstrated that five genotypes based on RdRp gene diversity were identified,including GⅡ.Pe,GⅡ.Pl2,GⅡ.P16,GⅡ.P17,and GⅡ.P7,while six genotypes based on VP1 gene were identified,including GⅡ.4 Sydney 2012,GⅡ.3,GⅡ.2,GⅡ.1,GⅡ.17,and GⅡ.6.Conclusion There were various sub-genotypes of NoV GⅡ among infected population in Ningxia region from 2016 to 2017.GⅡ.Pe and GⅡ.P12 were predominant sub-genotypes in RdRp region while GⅡ.4 Sydney 2012 and GⅡ.3 were in VP1 region.
