摘 要:目的了解湖南省美沙酮门诊就诊者感染丙型肝炎病毒(HCV)基因型特征。方法收集湖南省内14个市州18岁以上、未接受抗病毒治疗、既往HCV抗体阳性的美沙酮门诊患者的血浆样本,每市州样本15份,共210份。对收集到的血浆样本分别进行HCV抗体ELISA复检、荧光PCR法定量HCV-RNA检测和基因分型检测。结果在210名调查对象中,ELISA法复检HCV抗体阳性标本202例,阳性率96.19%;荧光PCR法检出HCV-RNA病毒载量>15 IU/ml标本181例,阳性率86.19%。HCV基因型分别为1型18例(9.94%),2/3型51例(28.18%),1型混合2/3型47例(25.97%),其他型别65例(35.91%)。区域分布以非1型为主,2/3型普遍分布。结论湖南省HCV基因型比较复杂,有待深入研究。Objective To investigate the characteristics of HCV genotypes of outpatients in methadone maintenance treatment clinics in Hunan Province. Methods A total of 210 plasma samples were collected from outpatients aged above 18 years, without antiviral therapy and with positive anti-HCV antibody in methadone maintenance treatment clinics in 14 cities/prefectures in Hunan Province, and 15 plasma samples were collected from each city/prefecture. All the samples were tested with HCV antibody ELISA, HCV-RNA fluorescent quantitative polymerase chain reaction(FQ-PCR) method and genotyping assay. Results Among all the surveyed subjects, 202(96.19%) were positive for anti-HCV antibody reconfirmed by ELISA method, and 181(86.19%) were positive for HCV-RNA by FQ-PCR method, with the viral load of >15 IU/ml. There were 18(9.94%) cases of HCV genotype 1 infection, 51(28.18%) cases of HCV genotype 2/3 infection, 47(25.97%) cases of HCV mixed genotype 1 and 2/3 infection, and 65(35.91%) cases of other genotype infections. Regional distribution showed that the genotype non-1 was the main HCV genotype, and the genotype 2/3 was the common genotype. Conclusions HCV genotypes in Hunan Province are complex, and it needs to be further studied.
