补肾活血、解毒化浊法联合盐酸二甲双胍片治疗对肥胖型多囊卵巢综合征患者GLP-1、GLP-2、PYY的影响

摘　　要：目的:观察补肾活血、解毒化浊法对肥胖型多囊卵巢综合征(polycystic ovary syndrome,PCOS)患者胰岛素抵抗的改善作用及其对胰高血糖素样肽-1(GLP-1)、胰高血糖素样肽-2(GLP-2)、酪酪肽(PYY)的影响。方法:受试者为广西中医药大学第一附院医院门诊诊断为肥胖型PCOS患者80例,按随机数字表法分组,对照组40例采用盐酸二甲双胍片治疗;治疗组40例采用补肾活血、解毒化浊法联合盐酸二甲双胍片治疗。治疗前后空腹采血,测定血清FINS、FBG、T,FSH、LH、GLP-1、GLP-2、PYY水平,计算HOMA-IR值。结果:两组患者治疗后血清FINS、HOMA-IR、T、FSH,LH显著降低(P <0. 01),GLP-1、GLP-2、PYY显著升高(P <0. 01);与对照组比较;治疗组FINS、HOMA-IR、T、FSH、LH下降均更加明显(P <0. 05),而GLP-1、GLP-2、PYY升高更加明显(P <0. 05)。结论:补肾活血、解毒化浊法治疗可能通过升高肥胖型多囊卵巢综合征患者血清GLP-1、GLP-2、PYY水平,从而改善胰岛素抵抗和性激素水平。Objective: To observe the effects of tonifying kidney and activating blood and resolving toxing and turbid method on insulin resistance,GLP-1,GLP-2 and PYY in patients with obesity-induced polycystic ovary syndrome( PCOS). Methods:Eighty patients diagnosed as obese PCOS in the First Affiliated Hospital of Guangxi University of Chinese Medicine. Patients were divided into two groups according to random number table. Forty cases in the control group were treated with metformin. Forty cases in the treatment group were treated with tonifying kidney and activating blood and resolving toxing and turbid method combined metformin. Fasting blood samples were collected before and after treatment. The serum levels of FINS,FBG,T,FSH,LH,GLP-1,GLP-2 and PYY were measured and HOMA-IR values were calculated. Results: The FINS,HOMA-IR,T,FSH,LH levels in the two groups were significantly decreased after treatment( P < 0. 01). GLP-1,GLP-2 and PYY levels were significantly increased after treatment( P < 0. 01). Compared with the control group,the levels of the treatment group changed more significantly( P < 0. 05). Conclusion: Tonifying kidney and activating blood and resolving toxing and turbid method could elevate the levels of serum GLP-1,GLP-2 and PYY in patients with obese PCOS,and thus improving insulin resistance and sex hormone levels.
