防旋髓内钉与锁定钢板内固定治疗老年股骨粗隆间骨折的效果观察

摘　　要：目的探讨防旋髓内钉与锁定钢板内固定治疗老年股骨粗隆间骨折的效果。方法选择2015年4月至2016年9月我院收治的股骨粗隆间骨折老年患者94例,随机分为对照组和观察组各47例。对照组行锁定钢板内固定治疗,观察组行防旋髓内钉内固定治疗,比较两组患者的手术及预后相关指标、 Harris评分及并发症发生率。结果观察组的术中出血量、手术时间、骨折愈合时间、住院时间、并发症发生率均优于对照组(P <0.05)。观察组术后1个月、 3个月的Harris评分高于对照组(P <0.05)。结论与锁定钢板内固定相比,防旋髓内钉治疗老年股骨粗隆间骨折的效果较好,具有术中出血量少、手术时间短、骨折愈合快、术后并发症少等优点,可显著改善患者的髋关节功能,具有较高的应用价值。Objective To explore the effect of anti-rotation intramedullary nail and locking plate internal fixation in the treatment of senile femoral intertrochanteric fracture. Methods 94 cases of elderly patients with femoral intertrochanteric fracture admitted to our hospital from April 2015 to September 2016 were selected and randomly divided into control group and observation group, with 47 cases in each group. The control group received locking plate internal fixation, and the observation group was treated with anti-rotation intramedullary nail internal fixation. The operation and prognosis related indicators, Harris score and incidence of complications were compared between the two groups. Results The intraoperative blood loss, operation time, fracture healing time, hospitalization time and incidence of complications of the observation group were better than those of the control group(P<0.05). The Harris scores 1 month and 3 months after operation of the observation group were higher than those of the control group(P<0.05). Conclusions Compared with locking plate internal fixation,anti-rotation intramedullary nail in the treatment of senile femoral intertrochanteric fracture has better effect and the advantages of less blood loss, shorter operation time, faster fracture healing, and fewer postoperative complications, and can significantly improve patients’ hip joint function, with higher application value.
