摘 要:目的回顾并讨论硅胶、膨体聚四氟乙烯、羟基磷灰石颗粒及透明质酸钠凝胶隆鼻的优缺点,并对其安全性进行评价。方法收集我科2012年3月-2018年2月隆鼻手术患者552例。隆鼻材料采用硅胶假体386例(其中联合耳软骨移植垫鼻尖31例),膨体聚四氟乙烯80例(其中联合耳软骨垫鼻尖18例),羟基磷灰石颗粒52例(其中17例为二次鼻整形),透明质酸钠凝胶34例。结果硅胶假体隆鼻患者中,2例隆鼻术鼻小柱右缘切口愈合欠佳假体外露,4例鼻背外形歪斜,3例假体外观明显,无假体感染病例出现。膨体聚四氟乙烯隆鼻患者中,1例出现感染;无假体歪斜、鼻根部假体浮动出现。羟基磷灰石颗粒隆鼻患者中,1例切口未愈合;1例矫正不满意;1例因鼻尖植入颗粒分散出现鼻头圆钝;无感染、皮肤破溃、鼻形歪斜等并发症出现。透明质酸钠凝胶隆鼻患者中,1例患者对鼻背高度不满意。其余患者对手术效果表示满意。结论在隆鼻材料的选择中需要根据材料的生物相容性、抗感染能力以及可塑性方面的优缺点进行合理选择。Objective To investigate and discuss the advantages and disadvantages of silicone implant,e-PTFE,calcium hydroxyapatite and hyaluronic acid for augmentation rhinoplasty.Methods From March 2012 to February 2018,we adopted these four types of materials in 552 patients for augmentation rhinoplasty.386 cases with silicone implant,31 patients combined with auricular cartilage for the tip augmentation.18 of 80 patients who were implanted e-PTFE implants underwent the auricular cartilage for tip augmentation.17 of 52 patients who were implanted with calcium hydroxyapatite were secondary rhinoplasty and 34 patients were injected with hyaluronic acid.Results Among the patients with silicone implant,implant exposed through the unhealed incision in 2 cases,4 cases of asymmetric implant contour,3 cases of obvious post-operative traces,and no infection occurred.1 case of infection occurred in the patients with e-PTFE,without asymmetric contour and implant floating appearing.Among the patients with calcium hydroxyapatite,there were 1 case of incision unhealed,1 unsatisfied case and 1 case of tip bulb for the implanting particles dispersion,without other complications like infection,skin break through,and so on.One of the patients with hyaluronic acid was unsatisfied with the dorsal height.The rest of the patients were all satisfied with the results.Conclusion during the augmentation rhinoplasty,we should choose the proper implanting material based on the biocompatibility,antibiotic property and plasticity.
