乳晕菱形真皮瓣支撑填充内陷乳头术围手术期观察及护理

摘　　要：目的总结乳晕菱形真皮瓣支撑填充内陷乳头术围手术期的临床观察及护理措施。方法 2017年1月～2018年10月我院共实施乳晕菱形真皮瓣支撑填充术治疗乳头内陷79例(140侧乳头),对其实施术前宣教宣教及心理护理、术中护理配合、术后临床观察、并发症防治及护理等系统化护理;随访6～12个月,以评价疗效及护理质量。结果术后12侧乳头发生轻度感染,局部红肿明显,发生率为8.57%,,经对症处理5～12d均好转;3例表现双侧乳头不对称,发生率为2.14%;无乳头坏死、皮瓣坏死、切口裂开等严重并发症发生;术后6～12个月随访,140枚乳头均外观挺拔饱满,乳晕清晰,瘢痕不明显,17例患者可进行正常哺乳,患者对护理服务满意。结论乳晕菱形真皮瓣支撑填充内陷乳头术具有切扣微小隐蔽,对乳晕及乳头敏感性破坏小,可保留哺乳功能等优点,是一项安全有效的乳头内陷治疗方法,在乳晕菱形真皮瓣支撑填充内陷乳头术围手术期开展系统化护理,可有效缓解患者不良心理情绪,促进身心恢复,提升护理质量。Objective To summarize the perioperative clinical observation and nursing measures of the areola-shaped dermal flap support filling invaginal papilla.Methods From January 2017 to October 2018,a total of 79 cases of nipple retraction(140 nipples)were treated with areola-shaped dermal flap support.The pre-operative missionary education and psychological nursing,intraoperative nursing cooperation and surgery were performed.Systematic nursing after post-clinical observation,prevention and treatment of complications,and nursing;follow-up for 6 to 12 months to evaluate efficacy and quality of care.Results Mild infection occurred in 12 nipples after operation,local redness and swelling were obvious,the incidence rate was 8.57%,and improved by symptomatic treatment for 5-12 days;3 cases showed bilateral nipple asymmetry,the incidence rate was 2.14%;no papillary necrosis,skin Severe complications such as petal necrosis and incision rupture occurred.After 6 to 12 months follow-up,140 nipples were full and full,the areola was clear,and the scar was not obvious.17 patients could be breast-feeding normally,and the patients were satisfied with the nursing service.Conclusion The areola diamond-shaped dermal flap-filled invaginal nipple has the advantages of small concealment,small damage to the areola and nipple,and the ability to retain breast-feeding function.It is a safe and effective treatment method for nipple retraction in the areola diamond-shaped leather.Systematic nursing during the perioperative period of the valve support filling invaginal nipple can effectively alleviate the patient’s bad psychological emotions,promote physical and mental recovery,and improve the quality of care.
