76例小儿急性单侧下肢迟缓性麻痹病因及临床分析

摘　　要：目的:探讨小儿急性单侧下肢迟缓性麻痹病因、临床特征、治疗及预后。方法:回顾性分析2015年6月至2017年3月河南省省立医院和郑州市儿童医院就诊的76例单侧下肢迟缓性麻痹患儿临床资料并总结、分析。结果:小儿急性单侧下肢迟缓性麻痹病因谱广,主要以病毒感染及一过性髋关节滑膜炎为主,预后好。迁延不愈患儿,注意排除腹部占位、脊髓占位、出血、血液病、布氏菌感染等特殊病因,给予限制运动,应用能量合剂,丹参静脉滴注改善微循环,抗病毒治疗以及短期应用糖皮质激素抗炎治疗后显效58例,有效16例,无效2例,3个月后复查肌力,75例均恢复正常,1例患儿出现跛行。结论:小儿急性单侧下肢迟缓性麻痹体格检查及病史应详尽,避免误诊,及时诊断,辨明原因,经对症治疗后,绝大部分肌力可恢复正常,预后较好。Objective To investigate the cause of acute unilateral flaccid paralysis of the lower limb of the children, clinical characteristics, treatment and prognosis. Methods Analyze the clinical date of children with unilderal flaccid paralysis of the lower limb who were diagnosed in Henan Provincial Hospital and Zhengzhou Children’s Hospitalfrom June 2015 to March2017 retrospectively. Results The cause of acute unilateral lower extremity palsy in children is broad, mainly viral infection and transient hip synovitis, and the prognosis is good. Children who have not healed, pay attention to the exclusion of abdominal occupying, spinal cord occupying, bleeding, blood disease, brucellosis infection and other special causes, give restricted exercise,apply energy mixture, Danshen intravenous drip to improve microcirculation. After antiviral treatment and short-term application of glucocorticoid anti-inflammatory treatment, 58 cases were effective, 16 cases were effective, 2 cases were ineffective, muscle strength was reviewed after 3 months, 75 cases were normal, and 1 case was claudication. Conclusion The physical examination and medical history of unilateral flaccid paresis of the lower limb of the children should be exhaustive, avoid misdiagnosis, diagnose timely and treat effectively, the clinical prognosis is good.
