自拟温经通络泡洗方联合西药治疗2型糖尿病周围神经病变30例临床观察

摘　　要：目的观察自拟温经通络泡洗方联合西药治疗2型糖尿病周围神经病变的临床疗效及安全性。方法将60例患者按随机数字表法分为治疗组和对照组各30例,治疗组给予基础治疗+自拟温经通络泡洗方泡洗,对照组给予基础治疗+温水泡洗,其中基础治疗包括常规降血糖、降血压、降血脂、饮食运动及口服甲钴胺片等;泡洗每日早晚各1次,每次20～30 min,连续进行20天。观察治疗后两组患者临床疗效,中医证候积分、多伦多临床评分系统(TCSS)评分以及电流感觉阈值(CPT)的变化情况,并观察不良反应。结果治疗组临床疗效总有效率为76. 67%,对照组为46. 67%,治疗组高于对照组(P <0. 05);治疗后两组中医证候积分、TCSS评分,双侧正中神经、腓浅及腓深神经CPT值均较治疗前明显降低(P <0. 05),且治疗后治疗组中医证候积分、TCSS评分及双侧正中神经、腓浅及腓深神经CPT值均明显低于对照组(P <0. 05)。两组患者血常规、肝功能、肾功能、心电图治疗前后未见明显变化。结论自拟温经通络泡洗方联合西药治疗2型糖尿病周围神经病变的临床疗效较好,未见不良反应。Objective To observe the clinical effects and safety of self-made Wenjing Tongluo Soaking and Washing Prescription(温经通络泡洗方)combined with western medicine in treating type 2 diabetic peripheral neuropathy.Methods A total of 60 patients were randomly divided into treatment group and control group,with 30 cases in each group.The treatment group was given basic treatment plus self-made Wenjing Tongluo Soaking and Washing Prescription,while the control group was given basic treatment plus warm water soaking and washing.The basic treatment included routine hypoglycemia,hypotension,lipid lowering,dietary regulation,exercise and oral administration of Mecobalamin tablets.The soaking and washing was given once in the morning and evening,20 to 30 min each time,for 20 days.The clinical efficacy,changes of traditional Chinese medicine(TCM)syndrome score,Toronto clinical scoring system(TCSS)and current perception threshold(CPT)of both groups,as well as the adverse reactions were observed.Results The total effective rate was 76.67%in the treatment group,higher than46.67%in the control group(P<0.05).After treatment,the TCM syndrome scores,TCSS and the CPT values of bilateral median nerve,superficial temporal and deep sacral nerve of both groups were significantly lower than those before treatment(P<0.05),and those in the treatment group after treatment was significantly lower than those in the control group(P<0.05).There were no significant changes in blood routine,liver function,renal function and electrocardiogram after treatment in both groups.Conclusion The self-made Wenjing Tongluo Soaking and Washing Prescription combined with western medicine has a good clinical efficacy in the treatment of type 2 diabetic peripheral neuropathy,without adverse reactions.
