老年慢性心力衰竭患者家庭照顾者负担及影响因素分析

摘　　要：目的探讨老年慢性心力衰竭患者家庭照顾者负担及其影响因素。方法采用便利抽样法,选择2016年1月—2017年6月哈尔滨医科大学附属第一医院心内科就诊的老年慢性心力衰竭患者235例及其主要照顾者为研究对象。采用患者及照顾者一般资料调查表、心力衰竭患者自我管理量表以及Zarit负担量表(ZBI)对患者及其照顾者进行问卷调查。本研究共发放各调查问卷235份,回收有效问卷208份。结果208名慢性心力衰竭患者照顾者ZBI总分(33.6±10.4)分,照顾者整体处于轻度负担水平。慢性心力衰竭患者的年龄、费用支付方式、心功能分级、自我管理水平能够影响照顾者负担水平(P<0.05)。照顾者的年龄、居住情况、工作情况及每日照顾时间能够影响其负担水平(P<0.05)。结论老年慢性心力衰竭照顾者存在照顾负担,医务工作者应该根据患者家庭具体情况,对患者照顾者进行个体化的专业支持和人文关怀,进而改善照顾者整体负担水平。Objective To investigate the burden of family caregivers in elderly patients with chronic heart failure(CHF)and its influencing factors.Methods A total of 235 elderly patients with CHF and their caregivers from Cardiology Department,the First Affiliated Hospital of Harbin Medical University were surveyed by questionnaire using the general information scale of patients and caregivers,Self-Management Ability Scale of Heart Failure Patients and Zarit Caregiver Burden Interview.In this study,235 questionnaires were distributed and 208 valid questionnaires were collected.Results The total ZBI score of the 208 patients with CHF was(33.6±10.4),with an overall moderate burden.The burden level of caregivers can be affected by age,way of payment,cardiac function classification and self-management level of patients with CHF(P<0.05).Caregivers'age,living conditions,working conditions and daily care time can affect their burden level(P<0.05).Conclusions Elderly patients with CHF have a relatively high overall burden.Medical staff should provide individual professional support and humanistic care to caregivers according to the specific conditions of patients'families,so as to improve the overall burden level of caregivers.
