摘 要:目的:探究小儿髋关节滑膜炎高频率超声波(HFUS)检查的图像特征及动态监测的临床价值。方法:选取我院疑似小儿髋关节滑膜炎患儿88例作为研究对象,就诊时均行双侧髋关节HFUS检查,并于治疗期间重复检查,1次/周,直至患儿康复出院,分析HFUS检查的图像特征及动态监测价值。结果:HFUS检查结果显示,本组88例研究对象,小儿髋关节滑膜炎84例,共计患髋88个,检测图像均显示股骨颈颈前间隙增大,且>6mm。HFUS动态监测结果显示,单纯髋关节囊肿型患儿较髋关节腔积液型患儿康复快;病程较长及反复发作者以滑膜增生为主,患肢髋关节血流信号较丰富,血流速度较快,更改治疗方案后HFUS检查结果恢复正常。结论:小儿髋关节滑膜炎HFUS检查的图像特征主要为髋关节腔积液及滑膜增厚,治疗期间采用HFUS进行动态监测有助于观察治疗效果,能有效指导治疗方案的制定,具有较高应用价值。Objective:To explore the image characteristics and clinical value of dynamic monitoring of HFUS examination in children with synovitis of hip joint.Methods:88 cases of children with hip joint synovitis in our hospital were selected as the subjects.All patients underwent bilateral hip HFUS examination and repeated examinations during the treatment period,1 time / week until the child was discharged from hospital.Image characteristics and dynamic monitoring value of HFUS examination were analyzed.Results:The results of HFUS examination showed that there were 84 cases of hip synovitis in those 88 objectives and 88 diseased hip joints in 84 cases of hip synovitis.The examination images showed that the anterior cervical space of colluni femoris was enlarged,and more than 6mm.HFUS dynamic monitoring showed that children with simple hip cyst recovered faster than those with hip joint effusion.Patients with long disease course and recurrent attack were mainly synovial hyperplasia.The blood flow signal of the hip joint was more abundant and the blood flow velocity was faster.The results of HFUS examination returned to normal after changing the treatment plan.Conclusion:The characteristics of HFUS examination of hip synovitis in children are mainly the fluid and synovial thickening of the hip joint.Dynamic monitoring of HFUS during the treatment is helpful to observe the therapeutic effect,and can effectively guide the formulation of the treatment plan,which has high application value.
