摘 要:目的探讨慢性阻塞性肺疾病(COPD)患者血清和诱导痰中NOD样受体热蛋白结构域相关蛋白3(NLRP3)炎性小体的表达及临床意义。方法选取60例COPD患者(稳定期30例、急性加重期30例)和30例健康体检者(对照组),采用ELISA法检测血清和诱导痰中NLRP3炎性小体、IL-1β、IL-18水平,分析NLRP3炎性小体水平与诱导痰巨噬细胞、中性粒细胞、淋巴细胞百分比的相关性。结果稳定期、急性加重期患者血清和诱导痰中NLRP3炎性小体及IL-1β、IL-18水平均较对照组升高,且急性加重期患者诱导痰和血清中上述指标较稳定期升高(P<0.05)。稳定期患者巨噬细胞百分比与血清中NLRP3炎性小体、IL-1β、IL-18水平呈正相关,中性粒细胞百分比与诱导痰、血清中NLRP3炎性小体、IL-1β、IL-18水平呈正相关(P<0.05)。急性加重期患者巨噬细胞、中性粒细胞、淋巴细胞百分比与诱导痰和血清中NLRP3炎性小体、IL-1β、IL-18水平呈正相关(P<0.05)。结论 COPD患者血清和诱导痰中NLRP3炎性小体水平升高,与肺组织炎症反应密切相关。Objective To investigate the expression and clinical significance of NOD-like receptor pyrin domain containing 3(NLRP3)inflammasome in serum and induced phlegm of the patients with chronic obstructive pulmonary disease(COPD).Methods The levels of NLRP3 inflammasome,IL-1βand IL-18 in serum and induced phlegm were detected in 60 patients with COPD(30 cases with stable COPD and 30 cases with acute stage COPD)and 30 healthy people(group C).The relationship of NLRP3 inflammasome and proportion of macrophage,neutrophil and lymphocyte was analyzed.Results Compared with group C,the levels of NLRP3 inflammasome,IL-1βand IL-18 in serum and induced phlegm were higher in the patients with stable COPD and acute stage COPD,which were higher in the patients with acute stage COPD than those with stable COPD(P<0.05).The proportion of macrophage was positively correlated to serum levels of NLRP3 inflammasome,IL-1βand IL-18 in the patients with stable COPD(P<0.05).The proportion of neutrophil was positively correlated to serum and induced phlegm levels of NLRP3 inflammasome,IL-1βand IL-18 in the patients with stable COPD(P<0.05).The proportions of macrophage,neutrophil and lymphocyte were positively correlated to serum and induced phlegm levels of NLRP3 inflammasome,IL-1βand IL-18 in the patients with acute stage COPD(P<0.05).Conclusion The levels of NLRP3 inflammasome in serum and induced phlegm are higher in COPD patients,which is closely related to the inflammatory reaction of lung tissues.
