105例髋关节镜手术牵引相关并发症的回顾研究

摘　　要：背景:术中关节牵引是髋关节镜相对于其他关节镜技术的特殊之处,也是髋关节镜的技术应用难点之一,国内关于髋关节镜手术牵引相关并发症发生率的研究报道较少。目的:探讨髋关节镜手术牵引引起的相关并发症。方法:回顾性分析2012年4月至2016年12月行髋关节镜手术的105例患者的术后并发症情况,男42例,女63例;年龄21～53岁,平均(41.3±10.5)岁;手术时间为60～130 min,平均(105±32)min。分别于术后10 d和3个月进行随访,记录并分析牵引相关并发症情况。结果:105例患者全部获得随访。术后10 d随访时,足部损伤1例,膝关节疼痛1例,会阴部损伤1例,神经牵拉损伤1例,无一例发生深静脉血栓、肺栓塞、跛行、股骨头坏死、股骨颈应力性骨折,牵引并发症发生率为3.8%(4/105)。术后3个月随访时,牵引并发症均消失。结论:髋关节镜手术牵引相关并发症发生率较低,是一种安全的手术方法。术中应尽量缩短牵引时间、减少牵引重量,以减少并发症的发生。Background:Intraoperative articular traction is a special feature of hip arthroscopy compared with other arthroscopic techniques,and it is also one of the difficulties in the application of hip arthroscopy.There are few reports on the incidence of traction-related complications in hip arthroscopic surgery in China.Objective:To investigate the complications associated with traction during hip arthroscopic surgery.Methods:The complications of 105 patients who underwent hip arthroscopic surgery from April 2012 to December 2016 were retrospectively analyzed.There were 42 males and 63 females,aging from 21 to 53 years,with an average age of(41.3±10.5)years,and the operation time was 60-130 minutes,with an average of(105±32)minutes.The complications related to traction were recorded and analyzed at 10 days and 3 months after operation.Results:All patients were followed up.At 10 days follow-up,there were 1 case of foot injury,1 case of knee pain,1 case of perineum injury and 1 case of nerve traction injury.No deep venous thrombosis,pulmonary embolism,claudication,femoral head necrosis and stress fracture of femoral neck occurred.The incidence of traction complications was 3.8%(4/105).After 3 months follow-up,traction complications disappeared.Conclusions:Hip arthroscopy is safe with a low incidence of tractionrelated complications.During the operation,the traction time should be shortened and the traction weight should be reduced to reduce the occurrence of complications.
