摘 要:目的探讨高密度脂蛋白颗粒与冠状动脉粥样硬化性心脏病(冠心病)患者介入治疗后再次血运重建的关系。方法入选经皮冠状动脉介入治疗患者100例,1年后复查冠状动脉造影检查。其中36例患者需要进行再次血运重建(重建组),64例患者无需进行再次血运重建(对照组)。比较2组患者血脂及高密度脂蛋白颗粒大小与冠状动脉血运重建的关系。结果血运重建组患者Gensini积分较高(P<0.01),大颗粒HDL浓度减少(P<0.01),同时小颗粒LDL浓度增加(P<0.01)。Logistic回归分析发现,大颗粒HDL水平和血运重建呈负相关(OR=0.8,95%CI:0.7~0.9),而小颗粒LDL水平与血运重建呈正相关(OR=1.4,95%CI:1.1~1.8)。结论高密度脂蛋白颗粒和冠心病介入治疗后血运重建有关系,提示其在血运重建中起一定的作用。Objective To investigate the relationship between high-density lipoprotein particles and revascularization after interventional therapy in patients with coronary heart disease. Methods A total of 100 patients, who underwent percutaneous coronary intervention, were included in the study, and coronary angiography was performed 1 year later. Among the patients, 36 required revascularization(reconstruction group) and 64 did not require revascularization(control group). The relationship between serum lipid and high-density lipoprotein particle size and coronary revascularization was compared between the two groups. Results Compared with the control group, the Gensini score was higher(P<0.01), the concentration of HDL large particles decreased(P <0.01), and the concentration of LDL small particles increased(P <0.01) in the revascularization group. Logistic regression analysis showed that the level of HDL large particles was negatively correlated with revascularization(OR=0.8, 95% CI: 0.7,0.9), whereas the level of LDL small particles was positively correlated with revascularization(OR=1.4, 95% CI: 1.1,1.8). Conclusion High-density lipoprotein particles are associated with revascularization after interventional therapy for coronary heart disease, suggesting its certain effects in revascularization.
