脑卒中后吞咽障碍的中医康复疗法研究进展

摘　　要：本文采用"中医康复""吞咽障碍""脑卒中后""deglutition disorders""dysphagia""rehabilitation"等关键词对中国知网、pubmed等数据库进行检索,总结了近5年来中医康复疗法对脑卒中后吞咽障碍的研究进展,发现针灸治疗、中药内服、中药外用法、穴位按摩、穴位注射等具有中医特色的治疗方法在治疗脑卒中后吞咽障碍中通过不同的作用机制改善了吞咽肌群的运动,提高与吞咽功能相关的神经肌肉的兴奋性,促进受损神经功能的恢复,提高吞咽肌肉的力量、耐力和协调性,缓解废用性肌萎缩,改善其功能及咽部血流,实现吞咽反射弧的恢复与重建,从而提高吞咽摄食能力,为临床中医康复疗法治疗脑卒中后吞咽障碍提供了理论依据及新方法。This article uses the"rehabilitation of traditional Chinese medicine","swallowing disorder","poststroke","deglutition disorders","rehabilitation"and"dysphagia"as keywords for China's knowledge,such as pubmed database retrieval,summarized the rehabilitation therapy of traditional Chinese medicine in nearly five years to review the research progress of swallowing disorder after stroke,and found that acupuncture,oral administration and external application of Chinese medicine,Chinese traditional medicine usage,acupuncture point massage,acupuncture point injection therapy with TCM characteristics method in the treatment of swallowing disorders after stroke through different mechanisms to improve the increase swallowing muscle movement,improve the excitability of nerve muscles related to deglutition function,promote the recovery of damaged nerve function,improve the strength,endurance and coordination of deglutition muscle,alleviate the waste muscle atrophy,improve its function and pharyngeal blood flow,and achieve the recovery and reconstruction of the reflex arc of deglutition,thus improving the swallowing feeding ability.It provides a theoretical basis and a new method for the treatment of dysphagia after stroke.
