摘　　要：目的探讨新型哺乳体位装置对经阴道分娩产妇乳头皲裂发生率和纯母乳喂养率的应用效果。方法 2017年7-12月,便利抽样法选择南京市某三级甲等综合性医院产科经阴道分娩的初产妇为研究对象。按照住院号的单双数将其分为观察组和对照组(各145例)。对照组按照常规母乳喂养指导模式,观察组在对照组的基础上使用新型辅助哺乳体位装置,比较两组产妇乳头皲裂发生率、皲裂程度及纯母乳喂养率。结果产后42 d观察组乳头皲裂发生率及皲裂程度低于对照组,差异均有统计学意义(均P<0.05)。住院期间观察组纯母乳喂养率为51.7%,对照组为72.4%,差异有统计学意义(P<0.05);产后6个月随访,观察组纯母乳喂养率为86.2%,对照组为68.9%,差异有统计学意义(P<0.05)。结论辅助哺乳体位装置的使用可有效降低乳头皲裂的发生率,提高纯母乳喂养率,值得临床借鉴使用。Objective To explore the effect of a new type of breastfeeding posture device on the incidence of papillary chap and the rate of exclusive breastfeeding in parturients with vaginal delivery. Methods By convenience sampling method, the primiparas who delivered in the hospital from July to December 2017 were selected as the research objects. They were divided into the observation group and the control group, with 145 cases in each group. The control group was guided by routine breastfeeding instruction, while the observation group was given the new type of breastfeeding posture device on the basis of the control group. The incidence and degree of chapped nipples and exclusive breastfeeding rate were compared between the two groups. Results The incidence and severity of chapped nipples in the observation group were lower than those in the control group, and the difference was statistically significant (all P <0.05). In the hospital, the rate of breastfeeding was 51.7% in the observation group and 72.4% in the control group, and the difference was statistically significant ( P <0.05).Six months after postpartum, the rate of exclusive breastfeeding in the observation group was 86.2% and that in the control group was 68.9%,and the difference was statistically significant ( P <0.05). Conclusions The usage of breastfeeding posture device can effectively reduce the incidence of nipple chap and improve the rate of exclusive breastfeeding, which is worthy of clinical reference.
