关于社区卫生管理“医防融合”模式的研究

摘　　要：随着人们生活质量的提升,对身体健康的要求逐渐提高,而传统社区卫生管理工作已经无法满足当前居民对健康生活方式的需求,因此,在社区卫生管理的过程中,应重点关注老年人与慢性病居民的健康需求,探索最佳的卫生管理模式。目前,在社区卫生管理工作中,"医防融合"模式受到广泛关注,以维护居民健康为中心,以家庭医生签约服务为切入点,切实履行基本医疗和公共卫生服务职能,为居民提供健康管理服务。制定完善的健康干预方案,保证卫生服务工作的个性化发展,按照整体服务模式的要求为居民提供健康知识宣传教育服务,提升其疾病认知水平,进而提高服务依从性,为社区卫生管理工作的长远发展夯实基础。With the improvement of people's quality of life,the demand for health has gradually increased,while the traditional community health management can no longer meet the current residents'demand for a healthy lifestyle.Therefore,in the process of community health management,we should focus on the health needs of the elderly and residents with chronic diseases and explore the best health management mode.At present,in the community health management work,the"integration of medical care and prevention"model has received extensive attention,focusing on the maintenance of residents'health,taking family doctors'contracted services as the entry point,earnestly performing the basic medical and public health service functions and providing health management services for residents.Formulate a sound health intervention plan to ensure the personalized development of health services,provide residents with health knowledge publicity and education services in accordance with the requirements of the overall service model,improve their disease awareness level,and further improve service compliance to lay a solid foundation for the long-term development of community health management.
