膝关节急性与慢性前交叉韧带损伤疗效的对比研究

摘　　要：探讨前交叉韧带(ACL)损伤的治疗时机。随机选取80例ACL损伤患者,分为急性组(≤6周,40例)和慢性组(>6周,40例),均施行自体腘绳肌腱重建ACL,随访9～14个月,采用Lysholm评分、国际膝关节评分委员会(IKDC)评分、膝关节被动活动感知阈值,分别评估患者的主观感受、功能恢复情况、本体感觉。同时从磁共振(MRI)直接和间接影像对比分析术前急性与慢性ACL损伤的区别。急性和慢性ACL损伤在直接和间接影像上都有特征性表现;急性组与慢性组术后Lysholm评分、IKDC评分、膝关节被动活动感知阈值比较差异均有统计学意义(P<0.05)。对于有指征进行ACL重建的患者,均应尽早手术治疗。MRI提高对ACL损伤的精确诊断,并为临床治疗提供影像学依据。To investigate the opportunity of treatment of anterior cruciate ligament(ACL)injury.A total of 80 patient who had ACL injury were divided into acute group(≤6 weeks,40 cases)and chronic group(>6 weeks,40 cases).All patients were received autogenous hamstring tendon reconstruction for ACL.Followed-up for 9~14 months,the Lysholm Knee score,IKDC Knee score and Passive activity perception threshold were used to estimate patients’subjective feeling,situation of function recovery and proprioception.At the same time,the difference between acute and chronic ACL injury were analyzed by the direct and indirect digital radiograph of MRI before surgery.Acute and chronic ACL injury both displayed characteristic expressions in the direct and indirect image.There were significant differences in the Lysholm score,IKDC score,and knee passive activity perception threshold between the acute and chronic groups(P<0.05).For patients with indications for ACL reconstruction,surgery should be performed as soon as possible.MRI can improve the accurate diagnosis of ACL injury,also it can provide image evidence for the clinic treatment.
