摘 要:目的 探究老年股骨粗隆间骨折伴骨质疏松患者术后早期给予唑来膦酸治疗的临床应用效果及安全性.方法 选取2013 年1 月~2016 年9 月收治的股骨粗隆间骨折伴骨质疏松术后老年患者90 例, 随机分为两组, 对照组行常规治疗, 观察组行早期唑来膦酸治疗, 比较腰椎骨密度、患侧髋部疼痛值、临床疗效及用药安全性.结果 治疗前, 两组腰椎骨密度比较差异无统计学意义(P >0. 05);治疗后6 及12 个月, 观察组腰椎骨密度明显高于对照组( P<0. 05);治疗后1、 6 及12 个月, 观察组与对照组患侧髋部疼痛值均下降, 且观察组髋部疼痛值明显低于对照组(P <0. 05);观察组治疗总有效率高于对照组(P<0. 05);两组用药安全性相比差异无统计学意义(P >0. 05).结论 术后早期给予唑来膦酸治疗老年股骨粗隆间骨折伴骨质疏松, 可增加椎骨密度, 缓解髋部疼痛, 提高临床疗效.Objective To investigate the clinical effects and safety of early postoperative administration of zoledronic acid in older patients with intertrochanteric fracture and osteoporosis. Methods 90 older patients with intertrochanteric femur fractures and osteoporosis from January 2013 to September 2016 were selected. The patients were randomly divided into the control group and the observation group. The control group received routine treatment. The observation group received early zoledronic acid treatment. The density of lumbar vertebra, the ipsilateral hip pain, the efficacy and safety were recorded. Results There was no significant difference in the bone density of the lumbar vertebra between the two groups ( P > 0. 05 ). At 6 and 12 months after treatment, the bone density of the lumbar vertebra was significantly higher in the observation group than that in the control group ( P < 0. 05 ). At 1 , 6 and 12 months after treatment, the pain values of hip pain were decreased in both group, the decreases in the observation group were more significant than in the control group (P <0. 05). The overall response rate in the observation group was higher than that in the control group ( P < 0. 05 ). There was no significant difference in safety between the two groups (P > 0. 05). Conclusion Early administration of zoledronic acid in older patients with intertrochanteric femur fractures and osteoporosis may increase the density of vertebral bone, relieve hip pain and improve the clinical efficacy.
