摘　　要：目的研究改良Bilhaut-Cloquet术对先天性复拇指畸形的矫治效果。方法选取我院2017年1月至2018年1月收治的先天性复拇指畸形患儿104例为研究对象,采用随机数字法将其分为对照组和观察组,各52例。对照组给予经典Bilhaut-Cloquet术治疗,观察组给予改良Bilhaut-Cloquet术治疗。比较两组指间关节轴线偏离、指间关节不稳定、末节伸直受限及拇指对掌功能受损等发生率,并比较两组患儿术后并发症发生情况及家属满意度。结果观察组患儿指间关节轴线偏离、指间关节不稳定、末节伸直受限及拇指对掌功能受损发生率均明显低于对照组,差异具有统计学意义(P<0.05);两组患儿并发症总发生率比较,差异无统计学意义(P>0.05)。观察组患儿家属满意度为88.46%,显著高于对照组的65.38%,差异具有统计学意义(P<0.05)。结论改良Bilhaut-Cloquet术矫正先天性复拇指畸形效果显著,其能明显改善患儿拇指的外观和功能,值得临床推广。Objective To study the corrective effect of modified Bilhaut-Cloquet operation on congenital thumb duplication deformity.Methods A total of 104 children with congenital thumb duplication deformity admitted in our hospital from January 2017 to January 2018 were selected as the research objects and divided into control group and observation group by random number method,with 52 cases in each group.The control group was treated with classical Bilhaut-Cloquet operation,while the observation group was treated with modified Bilhaut-Cloquet operation.The incidences of the interphalangeal joint axis deviation,instability of the interphalangeal joint,limitation of end extension and impairment of thumb opposition function were compared between the two groups.The postoperative complications and satisfaction of the family members of the two groups were compared.Results The incidences of interphalangeal joint axis deviation,instability of interphalangeal joint,limitation of end extension and impairment of thumb opposition function in the observation group were significantly lower than those in the control group,and the differences were statistically significant(P<0.05).There was no significant difference in the total incidence of complications between the two groups(P>0.05).The satisfaction of family members in the observation group was 88.46%,which was significantly higher than 65.38% of the control group,and the difference was statistically significant(P<0.05).Conclusion Modified Bilhaut-Cloquet operation is effective in correcting congenital thumb duplication deformity.It can significantly improve the appearance and function of thumb in children and is worthy of clinical promotion.
