某院剖宫产术药费分析及用药干预研究

摘　　要：目的分析某院剖宫产不合理用药情况,探索合理用药管理可行方法。方法分别随机抽取某院2017年5月至2017年7月(干预前)和2017年10月至2017年12月(干预后)的剖宫产手术病例345、322例,回顾性分析剖宫产术患者的药费结构和不合理用药现象。结果干预组患者的人均住院费用、药品费用、药费占比均低于对照组(P<0.05);干预组预防使用抗菌药物的人均费用、人均累计DDD均低于对照组,选药合理率高于对照组(P<0.05);干预N组患者的益母草注射液处方使用率最低,为25.7%;干预组患者的益母草注射液处方人均用量均低于对照组。结论通过对剖宫产不合理用药情况进行分析及干预,可显著改善医院药费结构和不合理用药现象,促进合理用药,为医院建立单病种付费模式、加强临床路径中的用药管理提供参考。Objective To analyze the irrational drug use situation of cesarean section in a hospital,and explore the feasible method for rational drug use administration.Methods A total of 345 and 322 cases of cesarean section in a hospital from May 2017 to July 2017(before intervention)and from October 2017 to December 2017(after intervention)were randomly selected respectively.The drug cost structure and irrational drug use in patients undergoing cesarean section were retrospectively analyzed.Results The per capita hospitalization cost,drug cost and drug cost ratio of the intervention group were lower than those of the control group(P<0.05).The per capita cost of antibiotics and cumulative DDD in the intervention group were lower than those in the control group,and the rational rate of drug selection was higher than that of the control group(P<0.05).The usage rate of motherwort injection prescription in the intervention group N was the lowest,which was 25.7%.The per capita dose of motherwort injection prescription in the intervention group was lower than that of the control group.Conclusion Through the analysis and intervention for irrational use of cesarean section,it significantly improve the drug cost structure and irrational drug use in the hospital,promote rational use of drugs,provide reference for hospital to establish a single disease payment model and strengthen medication management in the clinical pathway.
