摘 要:目的探讨Graves眼病患者与正常人之间以及Graves眼病不同期患者之间IL-2、IL-17表达的差异及其临床相关性。方法选取未经治疗的Graves眼病患者(GO组)54例,根据临床活动性评分(CAS)将Graves眼病患者分为活动期GO组28例(CAS≥3),非活动期GO组26例(CAS <3);同时选取未经治疗的同期非突眼Graves病患者(GD组)30例及健康体检的健康对照组30例。对比各组IL-2、IL-17水平差异,以及活动期GO患者经甲基强的松龙治疗前后IL-2和IL-17水平变化。结果 IL-2水平在GO组、GD组和健康对照组依次升高(P <0. 05);IL-17水平在GO组、GD组和健康对照组依次降低(P<0. 05);与非活动期相比,活动期GO患者IL-17水平升高(P<0. 05), IL-2水平无显著变化(P>O.05)。使用甲基强的松龙治疗12周后,活动期GO患者CAS降低(P<0.05),IL-17水平下降(P <0.05),IL-2水平无显著变化(P>0.05)。结论 IL-2、IL-17在GO的发生发展中起重要作用,其中IL-17可望作为评价GO患者病情活动性的新指标。Objective To investigate the difference of IL-2 and IL-17 expression levels between Graves' ophthalmopathy patients and normal people and among patients with different stages of Graves' ophthalmopathy and their clinical correlations.Methods 54 patients with untreated Graves' ophthalmopathy(the GO group)were enrolled for this study.According to the clinical activity score(CAS),Graves' ophthalmopathy patients were divided into the active GO group with 28 cases(CAS ≥ 3)and the inactive GO group with 26 cases(CAS3).At the same time,30 patients with untreated unexpanded Graves' disease(the GD group),as well as 30 healthy controls(the control group)were also selected.The differences of IL-2 and IL-17 levels across groups and the changes of IL-2 and IL-17 levels in patients with active GO before and after the treatment with methylprednisolone were compared.Results The levels of IL-2 increased in the GO group,the GD group and the healthy control group(P <0.05).The IL-17 levels decreased in the GO group,the GD group and the healthy control group(P <0.05).Compared with the active period,the IL-17 level of patients with GO increased(P <0.05),and there was no significant change of IL-2 level(P >0.05).After 12 weeks of treatment with methylprednisolone,the CAS of active GO patients decreased(P <0.05),while IL-17 levels decreased(P <0.05),and IL-2 levels did not change significantly(P >0.05).Conclusion IL-2 and IL-17 play an important role in the development of Graves′ ophthalmopathy disease mechanism,and IL-17 is expected to be a new indicator for evaluating the activity of GO patients.
