摘　　要：目的探讨R波递增不良(poor R-wave progression,PRWP)及R波逆向递增(reversed R-wave progression,RRWP)的临床意义。方法回顾分析2018年9月至12月在皖南医学院第一附属医院心电图诊断为PRWP的患者183例(PRWP组)和RRWP的患者98例(RRWP组)。根据心脏彩超、X线胸片、冠状动脉血管造影结果,明确有无心室肥厚、心肌梗死等病变,并进行统计分析。结果通过χ~2检验,RRWP组中心肌梗死的发生率要高于PRWP组,差异有统计学意义(P <0. 05);两组间心室肥厚的发生率比较差异无统计学意义(P>0. 05); RRWP组正常者比例要低于PRWP组,差异有统计学意义(P <0. 05)。结论 PRWP及RRWP具有提示心肌梗死、心室肥厚的临床意义。RRWP更多见于心脏有器质性病变的患者,它提示心肌梗死的准确率要高于PRWP,在辅助诊断前间壁或前壁心肌梗死中有一定的临床实用价值。Objective To investigate the clinical significance of poor R-wave progression(PRWP)and reversed R-wave progression(RRWP).Methods Retrospective analysis was performed on 183 patients diagnosed with PRWP(PRWP group)and 98 RRWP patients(RRWP group)in the First Affiliated Hospital of Wannan Medical College from September 2018 to December 2018.Results of ultrasonic cardiogram,chest X-ray and coronary angiography were utilized to determine whether there was ventricular hypertrophy,myocardial infarction,and other lesions.Then statistical analysis was carried out on the data.Results Through chi-square test,the incidence of myocardial infarction in RRWP group was found higher than that in PRWP group,with statistically significant difference(P<0.05);the incidence of ventricular hypertrophy did not vary significantly between the two groups(P>0.05);the normal percentage of RRWP group was lower than that of PRWP group,with statistically significant difference(P<0.05).Conclusion PRWP and RRWP are clinically significant in suggesting myocardial infarction and ventricular hypertrophy.RRWP is more common in the patients with cardiac organic lesions.RRWP is more accurate than PRWP in indicating myocardial infarction,and has certain clinical practical value in making auxiliary diagnosis of antero-septal or anterior myocardial infarction.
