阿法骨化醇治疗糖尿病性骨质疏松症临床研究

摘　　要：目的探讨阿法骨化醇软胶囊联合胰岛素注射液治疗糖尿病性骨质疏松症疗效及对骨痛症状、骨代谢及血糖水平的改善作用。方法回顾性分析2014年6月—2018年4月收治的糖尿病并发骨质疏松92例的临床资料,按照治疗方法分为对照组和观察组,每组46例。对照组给予常规胰岛素,观察组在对照组基础上给予阿法骨化醇软胶囊。治疗2个月后对比2组骨痛症状、骨代谢、血糖水平及并发症情况。结果观察组治疗后骨痛评分和血清钙、磷水平低于治疗前和对照组,白介素-4水平高于治疗前和对照组(P <0. 05,P <0. 01)。2组治疗后空腹血糖均低于治疗前(P <0. 05)。观察组总有效率高于对照组,并发症发生率低于对照组(P <0. 05,P <0. 01)。结论阿法骨化醇软胶囊对骨痛、骨代谢症状有明显改善作用,但对血糖影响不大,与胰岛素联合应用,可在降血糖的同时,明显改善骨质疏松症状。Objective To investigate curative effect of Alfacalcidol Capsule combined with insulin injection in treatment of patients with diabetic osteoporosis and to improve symptoms of bone pain, bone metabolism and blood glucose levels. Methods Clinical data of 92 patients with diabetes complicated by osteoporosis admitted during June 2014 and April 2018 was retrospectively analyzed, and the patients were divided into control group ( n =46) and observation group ( n =46) according to different therapeutic drugs. Control group received routine insulin, while observation group was added with Alfacalcidol Capsule on the basis of treatment for control group. After two months of treatment, symptoms of bone pain, bone metabolism, levels of blood glucose and incidence rate of complications were compared in two groups. Results After treatment, in observation group, bone pain score and serum calcium and phosphorus levels were significantly lower, while interleukin-4 level was significantly higher than those before treatment and in control group ( P <0.05, P <0.01). Levels of blood-fasting sugar after treatment were significantly lower than that before treatment in two groups ( P <0.05). In observation group, the total effective rate was significantly higher, while incidence rate of complications was significantly lower than those in control group ( P <0.05, P <0.01). Conclusion Alfacalcidol Soft Capsule may obviously improve symptoms of bone pain and bone metabolism with little effect on blood sugar. Alfacalcidol Soft Capsule combined with insulin may significantly improve osteoporosis symptoms while lowering blood sugar simultaneously.
