MCV、MCH及RDW检测在小儿缺铁性贫血临床诊断价值的研究

摘　　要：目的探讨红细胞体积(MCV)、平均红细胞蛋白量(MCH)及红细胞分布宽度(RDW)检测在小儿缺铁性贫血诊断中的临床价值研究。方法选取2015年1月至2018年2月于本院就诊的缺铁性贫血患儿48例为IDA组,同时选取同期于本院体检的健康儿童48例为对照组,对比两组间MCV、MCH及RDW水平差异。结果 IDA组患儿的MCV、MCH水平显著低于对照组(P<0.05),RDW水平显著高于对照组(P<0.05)。48例IDA患儿MCV异常率85.42%,MCH异常率77.08%,RDW异常率89.58%。MCV、MCH及RDW联合检测的诊断准确性、敏感性、特异性显著高于单一指标检测(P<0.05)。结论 MCV、MCH及RDW联合检测能够有效提高小儿缺铁性贫血的诊断准确性、敏感性和特异性,在小儿缺铁性贫血诊断、鉴别及临床疗效评估中具有重要价值,适合实验条件受限的基层医院,值得在临床上推广使用。Objective To explore the clinical value of detection of MCV, MCH and RDW in the diagnosis of iron deficiency anemia in children.Methods 48 children with iron deficiency anemia in our hospital from January 2015 to February 2018 were selected as IDA group, and 48 healthy children in the same period were selected as control group. The differences of MCV, MCH and RDW levels between the two groups were compared.Results The levels of MCV and MCH in IDA group were significantly lower than those in control group(P<0.05), and the levels of RDW in IDA group were significantly higher than those in control group(P<0.05). The abnormal rates of MCV, MCH and RDW were 85.42%, 77.08% and89.58% in 48 IDA children. The diagnostic accuracy, sensitivity and specificity of combined detection of MCV, MCH and RDW were significantly higher than those of single index detection(P<0.05). Conclusion The combined detection of MCV, MCH and RDW can effectively improve the diagnostic accuracy, sensitivity and specificity of iron deficiency anemia in children. It has important value in the diagnosis, differential diagnosis and clinical evaluation of iron deficiency anemia in children. It is suitable for primary hospitals with limited experimental conditions and is worth popularizing in clinic.
