摘　　要：目的探讨采用免疫化学发光法并通过唐氏筛查对异常核型检出率与妊娠结局相关性分析。方法将2015年1月至2017年12月2 798例行唐氏筛查的妊娠中期妇女作为研究对象,对其进行回顾性分析。采用免疫化学发光法对全部妊娠中期妇女的3个指标,包括游离β人绒毛膜促性腺激素、游离雌三醇、血清甲胎蛋白进行检测,使用腾程孕中期母血清学产前筛查分析软件V1.0对风险值进行计算。结果唐氏筛查结果阳性193例,神经管缺陷筛查结果阳性35例,三体筛查阳性8例,筛查全阴性2562例,B超筛查孕周2795例;可以得出孕妇诊断阳性率比较差异无统计学意义;筛查高风险及低风险不良结局发生率分别为4.2%和3.1%,差异有统计学意义(P<0.05)。结论对妊娠中期妇女进行产前唐氏综合征筛查,是鉴别与诊断宫内先天性缺陷儿的重要措施,对后续预防措施的采取具有重要的指导意义。Objective To investigate the effect of abnormal karyotype detection rate on pregnancy outcomes by Down’ s screening. Methods We conducted a retrospective analysis of 2 798 routinely-diagnosed mid-pregnant women from January 2015 to December 2017. Immunochemiluminescence assay was used to detect all three characteristics of women in mid-gestational age, including free β-human chorionic gonadotropin, free estriol,and serum alpha-fetoprotein. Prenatal maternal serum maternal prenatal screening was used. Check the analysis software V1.0 to calculate the risk value. Results Down’ s screening results were positive in 193 cases, neural tube defects were positive in 35 cases, trisomy was positive in 8 cases,screening was negative in 2562 cases, and B ultrasound screening was 2795 cases;There was no significant difference in the positive rate of pregnant women. The incidence of high-risk screening and low-risk adverse outcomes was 4.2% and 3.1%, respectively. The difference was statistically significant(P<0.05). Conclusion Screening for prenatal Down’ s syndrome in mid-gestational women is an important measure for identifying and diagnosing intrauterine congenital defects, and it has important guiding significance for follow-up preventive measures.
