苯磺酸氨氯地平对老年单纯性高血压患者动脉硬化指数的影响

摘　　要：目的探索苯磺酸氨氯地平对老年单纯性收缩期高血压患者动脉硬化指数及颈动脉内膜-中膜厚度的影响。方法选取2017年5月至2018年4月该院收治的初诊为原发性单纯性收缩期高血压患者192例,按年龄大小分为中青年组(<60岁,107例)和老年组(≥60岁,85例)。治疗前、治疗12周后采用动态血压检查对比两组患者血压变化,动脉硬化检测测定两组患者动脉硬化指数包括肱踝脉搏波传导速度(baPWV)和踝臂指数(ABI),超声检查两组患者颈动脉内膜-中膜厚度。结果与青年组比较,老年组患者合并糖尿病发病率、baPWV、ABI均更高,而体重指数及舒张压更低,两组患者治疗后血压均较治疗前明显下降,baPWV、ABI均较治疗前明显改善;与中青年组比较,老年组患者治疗后颈动脉内膜-中膜厚度的改善更加明显。结论苯磺酸氨氯地平可改善老年单纯性高血压患者动脉硬化指数,且与中青年组比较,具有更强的颈动脉保护作用。Objective To investigate the effect of amlodipine besylate on arteriosclerosis index and carotid artery intima-media thickness in elderly patients with isolated systolic hypertension. Methods 192 patients newly diagnosed isolated systolic hypertension were investigated,after exclusion a total of 192 patients were enrolled in this study.All the patients were divided into two groups according to age:young group(age younger than 60 years old,107 cases),elder group (age greater than 60 years old,85 cases).Before and 12 weeks after treatment,ambulatory blood pressure test was used to compare the changes of blood pressure in two groups,arteriosclerosis indicators,included brachial ankle pulse wave velocity(baPWV) and ankle brachial index (ABI) were measured before and after treatment between the two groups.And the intima thickness of carotid artery was measured by ultrasonography in the two group. Results Compared with the young group,the incidence of diabetes mellitus,baPWV and ABI were higher in the elder group,while the BMI index and diastolic blood pressure were lower.After treatment,the blood pressure in the two groups decreased significantly,baPWV and ABI were significantly improved.In the elder group,the thickness of the carotid artery was improved more significantly. Conclusion Amlodipine can improve arteriosclerosis index in elderly patients with simple hypertension,and it has stronger carotid artery protection compared with young group.
