摘 要:目的分析德阳市医院就诊被诊断为感染性疾病儿童的EB病毒(EBV)感染情况,为预防EBV感染提供预警。方法将2013-02/2018-03四川德阳市医院收治被诊断为感染性疾病的住院与门诊患儿纳入研究,收集其临床资料与血液标本检验结果,分析患儿EBV感染情况及其所致疾病构成。结果共收集到1 650例1~14岁感染性疾病患儿资料,其中EBV累计感染患儿812例,新发EBV感染患儿153例,EBV累计感染率为49.21%,新发感染率为14.54%;9~14岁幼儿EBV累计感染率最高(60.43%),9~14岁幼儿EBV累计感染率明显高于1~3岁、4~8岁幼儿(P<0.05),自1~3岁幼儿开始,往后逐渐升高;新发感染率亦随幼儿年龄增长而升高,但各年龄段比较差异无统计学意义(P>0.05);1 650例患儿中,男性与女性年龄比较无明显差异(P>0.05),女性EBV累计感染率与新发感染率均明显高于男性(P<0.05);夏季EBV新发感染率明显低于春、秋、冬季(P<0.05);秋季EBV新发感染率明显高于春、夏、冬季(P<0.05);153例新发EBV感染患儿临床表现中,上呼吸道感染疾病、肺炎以及支气管炎占比分别达31.37%、22.88%和20.26%。结论德阳市医院就诊感染性疾病儿童中EBV感染较常见,女性高于男性,秋季更明显,EBV感染为呼吸道感染疾病重要病原体。Objective To analyze the Epstein- Barr virus (EBV) infection in children diagnosed with infectious diseases in Deyang Hospital and provide early warning for prevention of EBV infection. Methods Children patients diagnosed with infectious diseases from February 2013 to March 2018 in Deyang Hospital were included into this study. The clinical data and blood samples were collected to analyze the EBV infection and disease composition caused by EBV. Results The data of 1 650 children 1 to 14 years of age with infectious diseases were collected. Among them, 812 were diagnosed with EBV cumulative infection, and 153 with new EBV infection: the cumulative infection rate of EBV was 49.21%, and the new infection rate was 14.54%;the cumulative infection rate of EBV was the highest among 9 to 14years old children(60.43%), the cumulative infection rate of EBV in children aged 9 to 14 years was significantly higher than in children aged 1 to 3 years and 4 to 8 years old (P< 0.05) and gradually increased with age from 1 to 3 years. The new infection rate also increased with the age of children;there was no significant difference between age groups (P> 0.05). Among the I 650 children, there was no significant difference in the age between male and female patients (P>0.05), the cumulative infection rate and the new infection rate in females were significantly higher than in males (P < 0.05). The new infection rate of EBV in summer was significantly lower than in spring, autumn and winter (P < 0.05), and the incidence of new EBV infection in autumn was significantly higher than in other seasons(P<0.05). Among the clinical manifestation of 153 new children with EBV infection, upper respiratory tract infection, pneumonia and bronchitis accounted for 31.37%, 22.88%, and 20.26%, respectively. Conclusion EBV infection is common in children with infectious diseases in Deyang Hospital. The incidence is higher in females than in males and significantly high in autumn. EBV is an important pathogen for respiratory infections.
