摘 要:目的探讨血清25-羟维生素D_3[25(OH)D_3],微RNA-221(miRNA-221)水平与冠状动脉病变程度、内皮功能的关系。方法选取胜利油田胜利医院心内科收治的冠状动脉粥样硬化性心脏病(CHD)并行冠状动脉造影检查的患者86例作为CHD组,健康体检者30例作为对照组,根据冠状动脉病变计算Gensini积分,根据Gensini积分分为高分组26例,中分组41例,低分组19例,采用酶联免疫吸附法(ELISA)检测血清25(OH)D_3水平,通过实时荧光定量PCR检测血清miR-221水平,采用外周动脉张力技术(PAT)计算血管反应性充血指数(RHI)评价血管内皮功能,分析血清25(OH)D_3、miR-221水平与RHI、Gensini积分变化间的相关性。结果 CHD组血清miR-221水平明显高于对照组(P<0.05),血清25(OH)D_3、RHI明显低于对照组(P<0.05);高分组、中分组患者血清miR-221水平高于低分组(P<0.05),且高分组高于中分组(P<0.05);高分组、中分组患者血清25(OH)D_3、RHI低于低分组,高分组低于中分组(P<0.05);通过Pearson相关性分析显示,CHD患者血清25(OH)D_3、miR-221水平与年龄、血糖、低密度脂蛋白(LDL-C)无相关性(P>0.05);25(OH)D_3与RHI呈显著正相关(P<0.05),与Gensini积分呈显著负相关(P<0.05);miR-221与RHI呈显著负相关(P<0.05),与Gensini积分呈显著正相关(P<0.05);随着血清25(OH)D_3表达降低,miR-221表达水平呈明显升高趋势,25(OH)D_3与miR-221表达呈明显负相关(P<0.05);Logistic回归分析显示,年龄、miR-221是影响冠状动脉病变形成的危险因素(P<0.05),25(OH)D_3、高密度脂蛋白(HDL)是影响冠状动脉病变形成的保护因素(P<0.05)。结论 25(OH)D_3是CHD保护因素,miR-221是CHD的危险因素,与冠状动脉病变程度和RHI相关,血清25(OH)D_3降低与miR-221升高可能是冠状动脉病变程度和血管内皮功能损伤的预测指标。Objective To investigate the relationship between serum 25-hydroxyvitamin D 3[25(OH)D 3],miR-221 level and severity of coronary artery disease and endothelial function.Methods Eighty-six cases of coronary heart disease(CHD)patients underwent coronary angiography treated in the Shengli Oilfield Victory Hospital were selected as the CHD group and 30 healthy persons as the control group,the Gensini score was calculated according to the coronary artery disease.According to Gensini score,26 cases were divided into high score group,41 cases were divided into the middle score group and 19 cases were divided into the low score group,Serum 25(OH)D 3 level was detected by enzyme-linked immunosorbent assay(ELISA),and serum miR-221 level was detected by real-time fluorescent quantitative PCR.Peripheral arterial tension(PAT)technique was used to calculate vascular reactive hyperemia index(RHI)to evaluate vascular endothelial function,and the serum 25(OH)D 3 and miR-221 level,RHI and Gensini scores and their correlations were compared and analyzed.Results Serum miR-221 level in the CHD group was significantly higher than that in the control group(P<0.05),serum 25(OH)D 3,RHI were significantly lower than that in the control group(P<0.05).The serum miR-221 level of the high score group and the middle score group were higher than that of the low score group(P<0.05),the high score group was higher than the middle score group(P<0.05);the serum 25(OH)D 3,RHI of the high score group and the middle score group were lower than that of the low score group,the high score group was lower than the middle score group(P<0.05).Pearson correlation analysis showed that serum levels of 25(OH)D 3 and miR-221 in CHD patients were not associated with age,blood glucose,and low density lipoprotein(LDL-C),P>0.05,there was a significant positive correlation between serum 25(OH)D 3 and RHI in CHD patients(P<0.05),and a significant negative correlation between serum 25(OH)D 3 and Gensini score in CHD patients(P<0.05),there was a significantly negative co
