北京地区老年人外周血淋巴细胞亚群正常参考范围的建立及比较

摘　　要：目的建立及比较北京地区不同年龄段老年人外周血淋巴细胞亚群正常参考范围。方法将参与2018年度解放军总医院健康体检的524例北京地区健康成人按10岁为1年龄段共分为4组,采用美国Beckman流式细胞仪结合血液分析仪分别检测他们的外周血CD4^+、CD3^+及CD8^+等T淋巴亚群及CD19^+及NK细胞,采用SPSS 17.0软件来分别比较各组间各项指标的统计学差异,并采用受检人群中年龄≥60岁的老年人的检测结果来获得适用于北京地区老年人的淋巴细胞亚群正常参考范围。结果淋巴细胞百分比随年龄的增长有所降低; CD3^+、CD3^+CD8^+及CD4^+45RO^+占淋巴细胞百分比随着年龄的增长而增高;自然杀伤细胞(NK)占淋巴细胞百分比随着年龄的增长而降低。北京地区老年人外周血淋巴细胞亚群正常参考范围(以95%CI表示)如下:淋巴细胞:759～2 783 cell/μl(12.9～47.3);CD3^+:929～1610 cell/μl(53.08～91.92); CD3^+CD8^+:202～775 cell/μl(11.53～44.25); CD3^+CD4^+:250～1248 cell/μl(14.30～71.30); CD4^+45RA^+:33～662 cell/μl(1.9～37.80); CD4^+45RO^+:199～807 cell/μl(11.34～46.07); CD19:7～403 cell/μl(0.40～22.99); NK:32～501 cell/μl(1.80～28.61)。结论年龄因素可影响人外周血淋巴细胞亚群CD3^+、CD3^+CD8^+、CD4^+45RO^+及NK占淋巴细胞百分比,有必要建立适当的、适用于北京地区老年人的上述常见淋巴细胞亚群正常参考范围。Objective To establish and compare the referential range of peripheral blood lymphocyte subsets in elderly inhabitants with different age ranges in Beijing area.Methods 524 healthy adults participate in the deppartment of physical examination were divided into four groups according to their age ranges.Their peripheral blood CD4^+,CD3^+and CD8^+T lymphoid subgroups and CD19^+and NK cells were detected by American Beckman flow cytometry and hematology analysis.SPSS 17.0 was used to compare the statistical difference between these various indexes in each group.The referential ranges of lymphocyte subsets suitable for healthy elderly persons in Beijing were obtained from the test results of elderly persons over 60 years of age.Results The percentage of total lymphocytes is decreased with age range;CD3^+%,CD3^+CD8^+%and CD4^+45RO^+%have increased with age range;NK%have decreased with the increase of age range.The referential range of peripheral blood lymphocyte subsets of healthy elderly persons in Beijing(expressed in 95%confidential intervals)is as follows:lymphocytes:759~2 783 cells/μl(12.90%~47.3%);CD3^+:929~1610 cells/μl(53.08%~91.92%);CD3^+CD8^+:202~775 cells/μl(11.53%~44.25%);CD3^+CD4^+:250~1 248 cells/μl(14.30%~71.30%);CD4^+45RA^+:33~662 cells/μl(1.90%~37.80%);CD4+45RO+:199~807 cells/μl(11.34%~46.07%);CD19:7~403cells/μl(0.40%~22.99%);NK:32~501cells/μl(1.80%~28.61%).Conclusion Age factors may influence the percentage of lymphocyte subsets CD3^+,CD3^+CD8^+,CD4^+45RO^+and NK^+in human peripheral blood.It is necessary to establish the normal referential range of these common lymphocyte subsets suitable for the elderly inhabitants in Beijing area.
