摘　　要：目的探讨UACR水平与GDM患者肾脏损伤的相关性。方法选取2016年1月至2017年6月于石家庄市第二医院门诊孕检的孕妇185名,其中GDM患者85例(GDM组),血糖正常孕妇100名为非妊娠期糖尿病组(NGDM)。检测UACR、尿微量白蛋白(UmAlb)、β2微球蛋白(β2-MG)及UAlb水平。结果孕晚期时,与NGDM组比较,GDM组UACR、UmAlb和β2MG阳性率及水平升高(P<0. 05),GDM组中UACR阳性率高于UmAlb和β2MG阳性率(P<0. 05)。Pearson相关分析显示,GDM组UACR水平与UmAlb、β2MG及HbA1c呈正相关(r=0. 722,0. 583,0. 631,P<0. 05)。Logistic回归分析结果显示,UACR是GDM患者可能发生肾脏损伤的独立危险因素[OR(95%CI)1. 323(1. 129～1. 550),P<0. 05]。产后42 d GDM组UACR水平高于NGDM组(P<0. 05)。结论 GDM患者UACR水平升高,可能与GDM患者肾脏损伤相关。Objective To explore the association between the urine albumin creatinine ratio(UACR)level and the renal damage in patients with gestational diabetes mellitus(GDM).Methods Screening pregnant women who were examined in our hospital from Jan 2016 to Jun 2017,85 patients with GDM(GDM group)and 100 normal pregnant women(NGDM group)were included in this study.UACR,UmAlb,β2microglobulin(β2-MG)were measured.ResultsIn the third trimester,the positive rate and levels of UACR,UmAlb and β2MG were higher in GDM group than those in NGDM group(P<0. 05). In GDM group,the positive rate of UACR was higher than that of UmAlb and β2 MG(P<0. 05). Pearson correlation analysis showed that UACR levels were positively correlated with UmAlb,β2MG and HbA1 c in GDM group(r=0. 722,0. 583,0. 631,P<0. 05). Logistic regression analysis showed that UACR was an independent risk factor for renal injury in patients with GDM[OR(95% CI)1. 323(1. 129~1. 550),P<0. 05]. The levels of UACR in GDM group were higher than those in NGDM group at 6 weeks postpartum(P<0. 05).ConclusionIncreased UACR levels might be related to the kidney damage in patients with GDM.
