摘 要:目的探讨胎膜早破患者胎膜组织核苷酸结合寡聚化结构域受体2(NOD2)与基质金属蛋白酶-9(MMP-9)、活化型Caspase-3蛋白及羊水中肿瘤坏死因子-α(TNF-α)水平表达的相关性,为进一步了解胎膜早破的病因学及治疗途径提供新思路。方法选择足月胎膜早破、未足月胎膜早破、正常妊娠晚期妇女各30例,应用免疫组织化学染色法及蛋白印迹技术检测各组胎膜组织NOD2、MMP-9及活化型Caspase-3的蛋白表达部位及水平,ELISA法检测羊水中TNF-α水平,并分析四者表达的相关性。结果胎膜早破组胎膜组织NOD2、MMP-9、活化型Caspase-3表达及羊水TNF-α水平高于正常妊娠晚期组(P<0.05),未足月胎膜早破组NOD2表达及羊水TNF-α水平高于足月胎膜早破组(P<0.05),MMP-9及活化型Caspase-3在足月胎膜早破组胎膜组织中的表达与未足月胎膜早破组相比差异无统计学意义。胎膜组织中NOD2表达水平与羊水TNF-α水平、胎膜组织中MMP-9及活化型Caspase-3的表达呈正相关(均P<0.01)。结论 NOD2、TNF-α、MMP-9、活化型Caspase-3表达水平升高与胎膜早破的发生相关;四者在胎膜早破发病机制中存在关联。Objective To investigate the relationship between the expressions of nucleotide-binding oligomerization domain-containing 2(NOD2),matrix metalloproteinase-9(MMP-9)and activated Caspase-3 in the fetal membranes and TNF-αlevel in amniotic fluid from women with premature rupture of the membranes(PROM).Methods Thirty pregnant women with term premature rupture of the membranes(tPROM),30 cases of preterm premature rupture of the membranes(pPROM)and 30 cases of healthy term pregnancy(control group)were enrolled in the experiment.Immunohistochemistry was used to detect the location of NOD2,MMP-9 and activated Caspase-3 in the fetal membranes.Western blot assay was performed to analyze the expression levels of the three proteins.ELISA was used to detect the level of TNF-αin amniotic fluid.The relationship between the levels of the four proteins was also analyzed.Results The expression levels of NOD2,MMP-9 and activated Caspase-3 in fetal membranes and the level of TNF-αin amniotic fluid were significantly higher in PROM groups than those in control group(P<0.05).The levels of NOD2 and TNF-αwere significantly higher in pPROM group than those in tPROM group(P<0.05).There were no significant difference in the levels of MMP-9 and activated Caspase-3 between tPROM group and pPROM group.Furthermore,the level of NOD2 was positively correlated with the levels of TNF-α,MMP-9 and activated Caspase-3(P<0.01).Conclusion The elevated expression levels of NOD2,TNF-α,MMP-9 and activated Caspase-3 play an important role in PROM.NOD2,TNF-α,MMP-9 and activated Caspase-3 may play associative roles in the process of PROM.
