硝普钠与多巴胺对急性心力衰竭患者血流动力及肾功能的影响

摘　　要：目的:探讨分析急性心力衰竭(心衰)患者应用硝普钠联合多巴胺治疗对其血流动力学和肾功能的作用和影响。方法:将100例急性心衰患者作为研究对象并随机分组,对照组50例均予以常规方式联合硝普钠治疗,研究组50例在对照组基础上联合多巴胺治疗,对比两组的临床效果。结果:治疗后,两组MAP、HR、PaO_2/FiO_2、PVPI以及EVLWI、CVP等血流动力学指标均得以显著改善,差异有统计学意义(P<0.05),且研究组患者改善情况更加明显,差异有统计学意义(P<0.05);两组血清肌酐以及尿微量白蛋白尿肌酐等肾功能指标均得以显著改善,差异有统计学意义(P<0.05),研究组改善情况更加明显,差异有统计学意义(P<0.05);两组不良反应发生率均较低且相对接近,差异无统计学意义(P>0.05)。结论:针对急性心力衰竭患者应用硝普钠联合多巴胺实施治疗,能够有效改善血流动力学和肾功能,效果明显,安全性高,值得推广应用。Objective To probe on the effects of sodium nitroprusside and dopamine on patientsr blood flow dynamics and renal function in the treatment of patients with acute heart failure(AHF).Metliod 100 cases of the patients with AHF treated in our hospital were selected as research targets and were randomly divided into the control group(n=50)and the research group(n=50).The control group received conventional treatment of sodium nitroprusside.The research group received the joint treatment of sodium nitroprusside and dopamine.A comparison was made in clinical effects between the two groups.Results After treatment,the two groups underwent obvious improvement in their blood flow dynamics indexes such as HR,PaO2/FiO2,PVPI,EVLWI and CVP(P<0.05),and the improvement for the research group was better than that for the control group(P<0.05);the two groups also underwent obvious improvement in such renal function indexes as serum creatinine and microalbuminuria reatinine(P<0.05),and the improvement for the research group was better than that for the control group(P<0.05);the occurrence rates of adverse reaction for the two groups were low and close to each other(P>0.05).Conclusion The joint treatment of sodium nitroprusside and dopamine for patients with AHF can effectively improve patientsr blood flow dynamics and renal function.The therapy is both effective and safe,and is worthy of wide clinical application.
