皮肤悬吊牵引结合髋“人”字石膏固定治疗儿童股骨干骨折的疗效分析

摘　　要：目的:分析和探究皮肤悬吊牵引结合髋"人"字石膏固定治疗儿童股骨干骨折的临床疗效。方法:选择40例股骨干骨折患儿作为研究对象,将其随机分成对照组和研究组,每组20例。对照组采用闭合复位伸直位髋"人"字石膏固定治疗,研究组采用皮肤悬吊牵引结合屈髋屈膝45°髋"人"字石膏固定治疗,对比两组患儿的骨折愈合时间、负重时间、总体住院时间、治疗的总有效率、并发症发生率及重新手术率。结果:研究组患儿骨折愈合时间、负重时间均明显短于对照组,研究组患儿治疗期间并发症发生率、重新手术率明显低于对照组,两组对比差异有统计学意义(P<0.05),总有效率、总体住院时间两组间差异无统计学意义(P>0.05)。结论:与单纯的闭合复位伸直位髋"人"字石膏固定治疗相比,采用皮肤悬吊牵引结合屈髋屈膝45°髋"人"字石膏固定对股骨干骨折患儿进行治疗,不仅能有效提高患儿治疗的疗效和缩短其预后周期,同时,还能降低其不良并发症和再次手术发生率,因此,其对提高患儿预后质量具有重要的作用。Objective To analyze and explore the clinical efficacy of skin suspension traction combined with hip cast immobilization in the treatment of femoral shat fractures in children.Metliod Forty patients with femoral shat fractures were selected as study subjects.They were randomly divided into control group and study group with 20 patients in each group.The control group was treated with closed reduction and straightening.The patient was treated with cast immobilization of hips and herpes casts.The study group was treated with skin suspension traction combined with hip herniation plaster.The fracture healing time,weight-bearing time,total length of stay,total treatment effectiveness,incidence of complications and reoperation rate were compared between the two groups of children.Results The fracture healing time and weight-bearing time of the study group were significantly shorter than those of the control group.The incidence of complications and the rate of reoperation during the treatment period were significantly lower in the study group than in the control group.There was a significant difference between the two groups(P<0.05),there was no significant difference in total effective rate and total length of stay between the two groups(P>0.05).Conclusion Compared with the simple closed reduction and extended hip hips plaster cast,the treatment of children with femoral shat fracture using skin suspension traction combined with hip casts can not only effectively improve the curative effect and shorten the treatment of children.Its prognostic cycle,at the same time,can also reduce its adverse complications and the incidence of reoperation,therefore,it has an important role in improving the prognosis and quality of children.
