输尿管软镜下钬激光内切开引流术与后腹腔镜下囊肿去顶术治疗单纯性肾囊肿疾病的效果比较

摘　　要：目的探讨输尿管软镜下钬激光内切开引流术与后腹腔镜下囊肿去顶术治疗单纯性肾囊肿疾病的效果。方法选取2017年5月～2018年5月我院收治的64例肾囊肿患者作为研究对象,按照随机数字表法分为探究组(32例)与参照组(32例),探究组采用输尿管软镜下钬激光内切开引流术治疗,参照组采用后腹腔镜下囊肿去顶术治疗。比较两组的手术治疗情况及术后复发情况。结果探究组患者的治疗失血量少于参照组,手术治疗后出院用时和手术治疗用时显著短于参照组,差异均有统计学意义(P<0.05);两组患者的术后发热率和术后复发率比较,差异无统计学意义(P>0.05)。结论对于有肾囊肿疾病的患者,相比于后腹腔镜下囊肿去顶术,输尿管软镜下钬激光内切开引流术能加快手术进程,减少患者手术治疗过程中的失血情况,促进患者术后恢复,同时能达到与后腹腔镜下囊肿去顶术相似的术后发热率和复发率,推荐临床将输尿管软镜下钬激光内切开引流术应用于肾囊肿疾病的治疗中。Objective To investigate the effect of holmium laser internal incision and drainage under flexible ureteroscope and retroperitoneal laparoscopic cyst unroofing in the treatment of simple renal cyst diseases.Methods Sixty-four patients with renal cysts admitted to our hospital from May 2017 to May 2018 were selected as the study subjects,they were divided into the exploratory group(32 cases)and the reference group(32 cases)according to the random number table method.The exploratory group was treated with holmium laser internal incision and drainage under flexible ureteroscope,the control group was treated with retroperitoneal laparoscopic cyst unroofing.Operative treatment and recurrence were compared between the two groups.Results The treatment blood loss of the exploratory group was less than that of the reference group,and the time spent on discharge and surgical treatment after operation was significantly shorter than that of the reference group,the differences were statistically significant(P<0.05).There was no significant difference in postoperative fever rate and recurrence rate between the two groups(P>0.05).Conclusion For patients with renal cyst disease,compared with retroperitoneal laparoscopic cyst unroofing,flexible ureteroscopic holmium laser internal incision and drainage can speed up the operation process,reduce blood loss during the operation,promote the recovery of patients,and achieve similar postoperative fever and recurrence rate as retroperitoneal laparoscopic cyst unroofing.It is recommended that clinical use of flexible ureteroscopic holmium laser internal incision and drainage.Laser internal incision and drainage is used in the treatment of renal cyst diseases.
