摘　　要：目的分析胃镜喉罩在行内镜粘膜下剥离术(ESD)患者全麻中的应用效果。方法随机数字表法将117例择期行ESD全麻通气的患者分为观察组(n=57)与对照组(n=60),观察组行胃镜喉罩全麻通气,对照组则采取气管导管全麻通气。比较两组围术期心率(HR)、平均动脉压(MAP)水平,并统计两组自主呼吸恢复时间、拔管时间、麻醉后恢复室(PACU)时间及咽部不良反应情况。结果喉罩/气管导管置入即刻(T1)、喉罩/气管导管拔除即刻(T3)时HR、MAP水平观察组均显著低于对照组(P<0. 05);观察组自主呼吸恢复时间、拔管时间、PACU时间均显著短于对照组(P<0. 05);观察组咽痛、声嘶发生率均显著低于对照组(P<0. 05)。结论 ESD全麻通气中应用胃镜喉罩能有效稳定患者血流动力学,缩短拔管时间,减少咽部不良反应。Objective To analyze the effect of gastroscopic laryngeal mask on general anesthesia patients undergoing endoscopic submucosal dissection(ESD).Methods 117 patients who underwent elective ESD general anesthesia were randomly divided into observation group(n=57)and control group(n=60).The observation group underwent gastroscopic laryngeal mask general anesthesia,while the control group underwent tracheal catheter general anesthesia.The perioperative HR and MAP levels were compared between the two groups.The autonomous respiration recovery time,extubation time,PACU time,and adverse reactions in the pharynx of the two groups were compared.Results The HR and MAP levels in the observation group at T1 and T3 were significantly lower than those in the control group(P<0.05).The spontaneous breathing recovery time,extubation time and PACU time of the observation group were significantly shorter than those of the control group(P<0.05).The incidence of sore throat and hoarseness in the observation group was significantly lower than those in the control group(P<0.05).Conclusion The application of gastroscopic laryngeal mask in ESD general anesthesia can effectively stabilize hemodynamics,shorten extubation time and reduce pharyngeal adverse reactions.
