超声联合上消化道造影诊断婴幼儿先天性十二指肠梗阻的价值

摘　　要：目的探讨超声联合上消化道造影(UGI)检查诊断婴幼儿先天性十二指肠梗阻(CDO)的临床价值。方法回顾性分析经手术证实的我院116例可疑CDO患儿的超声和UGI结果。结果 UGI、超声和两者联合诊断CDO的准确率分别为96.55%、97.41%、100%;定位梗阻部位的准确率分别为88.66%、89.69%、94.84%;诊断病因总符合率分别为38.38%、60.61%、64.65%。结论超声能有效诊断婴幼儿CDO,并通过显示十二指肠周围毗邻组织增加病因诊断准确率。但对于超声诊断不明确的病例,建议联合UGI检查,有利于术前全面评估。Objective To investigate the clinical value of ultrasound combined with upper gastrointestinal series(UGI)in the diagnosis of congenital duodenal obstruction(CDO)with infants and young children.Methods 116 children undergoing both UGI and ultrasound for possible CDO confirmed by surgery were analyzed retrospectively.Results The accuracy rate of UGI,ultrasound and the combined diagnosis of CDO was 96.55%,97.41% and 100%,respectively;accuracy of locating the obstruction was 88.66%,89.69% and 94.84%,respectively;and etiological accuracy was 38.38%,60.61% and 64.65%,respectively.Conclusions The CDO can be effectively diagnosed by ultrasound,which can increase the etiological accuracy by showing adjacent tissue around the duodenum.The accuracy of diagnosis can be improved when ultrasound combined with UGI for the cases with unclear ultrosonic diagnosis,and benefit for comprehensive preoperative evaluation.
