摘　　要：目的分析连云港市麻疹发病的流行病学特征和健康人群免疫状况,为防治麻疹提供理论支持。方法对2008-2017年麻疹发病资料和免疫监测资料进行流行病学分析。结果 2008-2017年全市共报告麻疹病例1 383例,年均发病率3.03/10万;发病时间集中于1～3月,构成比达55.97%;高发年龄以0～7月龄组、8月～4岁组为主,分别占22.78%、47.00%,2010-2017年以15岁及以上和0～7月龄为主,分别占36.03%、35.29%;男性多于女性,构成比为63.49%、36.51%;职业以散居儿童为主,占68.91%,其次为学生12.65%。2013-2015年共采集健康人群血清标本859份检测麻疹抗体水平,阳性率83.70%。结论连云港市8月龄～40岁人群抗体阳性率高,均超过90%。麻疹发病呈明显的季节性,2010年以后麻疹发病处于低发水平,主要为成人病例和8月龄以下儿童。维持高水平的疫苗接种率和及时接种率是麻疹防控的关键。Objective To analyze the epidemiological characteristics and immune level of measles in Lianyungang City,so as to provide a scientific basis for control of measles.Methods Incidence data of measles from 2008 to 2017 and immunization monitoring data were analyzed by epidemiological methods.Results From 2008 to 2017,1383 measles patients occurred,and the average incidence was 3.03/100 000.The cases were concentrated in January to March,and the ratio was 55.97%.Patients aged 0~7 months and 8 months^4 years old occupied the mostly proportion(22.78%and 47.00%,respectively).From 2010 to 2017,patients aged 15 years old and above,0~7 months occupied the mostly proportion(36.03%,35.29%,respectively).The cases happened mainly in male,and the proportion was 63.49%with 36.51%in female.Scattered children occupied the most ratio(68.91%),and the second was students(12.65%).Among 859 subjects collected in a healthy population from 2013 to 2015,the serum antibody positive rate was 83.70%.Conclusion Antibody levels in the population aged 8 months to 40 years old are high,and all above are 90%.Measles has the obvious seasonal characteristic.After 2010,the incidence of measles is low and mainly concentrated in adults and children under the age of 8 months.Maintaining high levels of vaccination rate and timely vaccination rate is the key to control of measles.
