间歇充气加压泵联合足底动静脉泵对预防老年卧床病人下肢深静脉血栓的临床研究

摘　　要：目的观察间歇性充气加压泵(IPC)联合足底动静脉泵(AV泵)对预防老年住院病人下肢深静脉血栓(DVT)的效果。方法选取2015～2016年在我院老年内科住院的168例老年病人为研究对象,随机分为对照组、间歇性充气加压泵观察组(IPC组)、足底动静脉泵观察组(AV组)及间歇性充气加压泵联合足底AV泵试验组(IPC+AV组),每组各42例,其中对照组予以DVT传统常规护理,其余3组在对照组基础上另加相应方法实施干预,干预终止后,比较各组D-二聚体水平以及DVT发生情况。结果 IPC+AV组、IPC组或AV组的D-二聚体检测水平明显低于对照组(P<0. 05),IPC+AV组D-二聚体水平明显低于其余3组(P<0. 05); IPC组、AV组以及IPC+AV组的DVT发生率明显低于对照组(P<0. 01); IPC+AV组DVT发生率明显低于其余3组(P<0. 01),IPC组与AV组组间差异不显著(P>0. 05)。结论 IPC联合AV能有效降低老年卧床病人下肢DVT发生的风险,临床上值得推广。Objective To observe the effects of intermittent pneumatic compression pump(IPC)combined with plantar arteriovenous pump(AV)in preventing deep vein thrombosis(DVT)in the elderly inpatients.Methods A total of 168 elderly inpatients from 2015 to 2016 were chosen as subjects,and they were randomly divided into four groups,control group(n=42),IPC group(n=42),AV group(n=42)and IPC+AV group(n=42).The conventional therapy and nursing care were given in all groups.In addition,the other three observation groups received the corresponding intervention method.The level of thrombus molecular markers(D-dimer)and the incidence rate of DVT were compared among the four groups.Results The level of D-dimer in three observation groups was significantly lower than that in control group(P<0.05),and was significantly lower in IPC+AV group than that in other three groups(P<0.05);The incidence rate of DVT in three observation groups was significantly lower than that in control group(P<0.01),and was significantly lower in IPC+AV group than that in other groups(P<0.01).There were no significant differences in the level of D-dimer and the incidence rate of DVT between IPC group and AV group(P>0.05).Conclusions IPC combined with A-V pump can effectively reduce the risk of DVT in the elderly bedridden inpatients,and is worthy of promotion in clinic.
