疏肝解郁胶囊辨证与辨病治疗轻-中度抑郁症的对比研究

摘　　要：目的通过流行病学调查明确轻-中度抑郁症的中医证型分布规律;观察疏肝解郁胶囊对轻-中度抑郁症的疗效及辨证与非辨证条件下疗效与不良反应的差异,明确舒肝解郁胶囊最佳适应证。方法 120例轻-中度抑郁症患者,随机分为观察组80例和对照组40例。根据中医辨证将观察组患者分为肝郁脾虚组43例与非肝郁脾虚组37例。观察组接受疏肝解郁胶囊联合舍曲林片治疗,对照组接受安慰剂胶囊联合舍曲林片口服治疗,治疗6周。比较各组患者治疗前,治疗第2、4、6周末汉密尔顿抑郁量表(HAMD)评分、中医症候积分及综合疗效。结果治疗第2、4、6周末观察组、肝郁脾虚组、非肝郁脾虚组及对照组HAMD评分、中医症候积分均较治疗前下降(均P<0.05);肝郁脾虚组HAMD评分、中医症候积分低于非肝郁脾虚组和对照组(均P<0.05)。治疗第4、6周末观察组HAMD评分、中医症候积分低于对照组(均P<0.05);治疗结束时疗效比较,观察组及肝郁脾虚组优于对照组(均P<0.05),肝郁脾虚组优于非肝郁脾虚组(P<0.05)。结论舒肝解郁胶囊对肝郁脾虚型轻-中度抑郁症患者安全有效,联合舍曲林片效果优于单用舍曲林片。Objective Through epidemiological investigation,to clarify the distribution regularity of TCM syndromes of mild to moderate depression.To observe the curative effect of Shugan Jieyu Capsule on mild to moderate depression and the differences of curative effect and adverse reaction between syndrome differentiation and non-syndrome differentiation,and to define the best indication of Shugan Jieyu Capsule.Methods A total of 120 patients with mild to moderate depression were randomly divided into observation group(80 cases)and control group(40 cases).According to TCM syndrome differentiation,the patients in the observation group were divided into 43 cases of liver depression and spleen deficiency group and 37 cases of non-liver depression and spleen deficiency group.The observation group was given Shugan Jieyu Capsule and Sertraline Tablet,while the control group was given placebo Capsule and Sertraline Tablet for 6 weeks.The scores of Hamilton Depression Scale(HAMD),scores of TCM symptoms and comprehensive effects were compared before treatment,at the 2nd,4th and 6th weekends of treatment.Results At the 2nd,4th and 6th weekends of treatment,HAMD score and TCM symptom score of observation group,liver-depression and spleen-deficiency group,non-liver-depression and spleen-deficiency group and control group were significantly lower than those before treatment(all P<0.05);HAMD score and TCM symptom score of liver-depression and spleen-deficiency group were lower than those of non-liver-depression and spleen-deficiency group and control group(all P<0.05).The HAMD score and TCM symptom score of the observation group were lower than those of the control group at the 4th and 6th weekends of treatment(P<0.05);the curative effect of the observation group and the liver-depression and spleen-deficiency group was better than that of the control group(all P<0.05),and the liver-depression and spleen-deficiency group was better than the non-liver-depression and spleen-deficiency group(P<0.05).Conclusion Shugan Jieyu Capsule is safe a
