腹腔镜下子宫切除术中全麻联合硬膜外麻醉的应用分析

摘　　要：目的观察腹腔镜下子宫切除术中应用单纯全身麻醉和全身麻醉联合硬膜外麻醉两种不同麻醉方式下的效果,分析其优越性。方法 40例拟行腹腔镜下子宫切除术的患者,按照自愿选择的原则随机分为单纯麻醉组(D组,20例)和联合麻醉组(U组,20例)。D组患者实施单纯气管插管全麻,U组患者实施气管插管全麻联合硬膜外麻醉。观察两组患者分别在两种麻醉方式下的麻醉效果。结果 U组患者在肌肉松弛程度、自主呼吸恢复时间、复苏时间、拔管时间方面明显优于D组,术者的满意度方面也明显优于D组,差异有统计学意义(P<0.05)。结论腹腔镜下子宫切除术中应用全身麻醉联合硬膜外麻醉,不仅仅是肌松效果好,而且自主呼吸恢复时间迅速,术后拔管快,复苏时间短,术者的满意度高,具有明显的优势。Objective To observe the effect of general anesthesia and general anesthesia combined with epidural anesthesia in laparoscopic hysterectomy and analyze its advantages. Methods Forty patients undergoing laparoscopic hysterectomy were randomly divided into simple anesthesia group (group D, 20 cases) and combined anesthesia group (group U, 20 cases) according to the principle of voluntary selection.Patients in group D received simple tracheal intubation general anesthesia, patients in group U received tracheal intubation general anesthesia combined with epidxaral anesthesia.To observe the anesthetic effect of two groups of patients under two different anesthesia modes. Results The degree of muscle relaxation, recovery time of spontaneous breathing, resuscitation time and extubation time of group U were significantly better than those of group D, and the satisfaction of the sxirgeon was also significantly better than that of group D, with statistical significance (P<0.05). Conclusion The application of general anesthesia combined with epidural anesthesia in laparoscopic hysterectomy is not only effective for muscle relaxation, but also has obvious advantages such as rapid recovery of spontaneous breathing, rapid extubation, short recovery time and high satisfaction of the operator.
