补虚平喘汤治疗老年COPD缓解期的效果及对患者细胞免疫功能的影响

摘　　要：目的研究补虚平喘汤治疗老年慢性阻塞性肺疾病(COPD)缓解期的效果及对患者细胞免疫功能的影响。方法将我院2014年6月至2018年6月收治的80例老年COPD缓解期患者纳入研究对象,以随机数表法分为观察组与对照组,各40例。对照组给予西医常规治疗,观察组在对照组基础上加用补虚平喘汤治疗,两组患者均连续用药2个月。对比两组治疗前后FEV_1%、PEF、FEV_1/FVC、TNF-α、IL-8、hs-CRP、CD3^+、CD4^+、CD8^+及CD4^+/CD8^+水平,另外观察两组用药不良反应发生情况。结果治疗后,观察组FEV1%、PEF、FEV_1/FVC水平显著高于治疗前及对照组(P<0.05);治疗后,观察组TNF-α、IL-8、hsCRP水平显著低于治疗前及对照组(P<0.05);治疗后,观察组CD3^+、CD4^+、CD4^+/CD8^+水平显著高于治疗前及对照组,CD8+水平显著低于治疗前及对照组(P<0.05)。两组患者用药期间均未见明显不良反应,肝、肾功能检测无明显异常。结论补虚平喘汤治疗老年COPD缓解期能够改善患者肺功能、炎症因子水平,提高患者免疫力,且安全性较高,具有较高的临床应用价值。Objective To study the Effect of Buxu Pingchuan decoction in the treatment of old patients with COPD in remission stage and its influence on cellular immune function. Methods Eighty elderly patients with COPD in remission period admitted in our hospital from June 2014 to June 2018 were included in the study objects. They were divided into observation group and control group by random number table method, with 40 cases in each group. The control group was treated with routine western medicine treatment, the observation group was treated with Buxu Pingchuan on the basis of the control group decoction. The levels of FEV1%, PEF, FEV1/FVC, TNF-α, IL-8, hs-CRP and T lymphocyte group (CD3+, CD4+, CD8+, CD4+/CD8+) before and after treatment were compared between the two groups. The adverse reactions of the two groups were also observed. Results After treatment, the levels of FEV1%, PEF and FEV1/FVC in the observation group were significantly higher than those before treatment and the control group (P<0.05). After treatment, the levels of TNF-α, IL-8 and hs-CRP in the observation group were significantly lower than those before treatment and the control group (P<0.05). After treatment, the levels of CD3+, CD4+, CD4+/CD8+ in the observation group were significantly higher than those before treatment and the control group, and the CD8+ level in the observation group was significantly lower than that before treatment and the control group (P<0.05). No obvious adverse reactions were observed during the administration of the two groups, and no obvious abnormalities were found in liver and kidney function tests. Conclusion Buxu Pingchuan decoction can improve pulmonary function, inhibit levels of inflammation factors and improve immunity of elderly patients with COPD in remission stage, and has high safety and clinical application value.
