三级医院医疗检查结果互认实施现状及影响因素分析

摘　　要：目的分析三级医院医疗检查结果互认现状及其影响因素,为政府决策提供科学依据。方法 2018年6—8月对三级医院医疗检查结果互认试点医疗机构的342例患者进行调查,运用逐步回归分析等方法对数据资料进行分析。结果仅有59.19%的患者对有关三级医院医疗检查结果互认的情况是有所听闻的,30.53%的患者不清楚有关三级医院医疗检查结果互认的情况,仅有5.61%的患者非常了解有关三级医院医疗检查结果互认的情况。不同医疗费用的患者对重复检查的态度不同,差异有统计学意义(P=0.000)。医方在采用患者原有病情检查资料的做法上仍持保守态度,超过50%的患者表示在就医过程中三级医院不采用原有医疗检查结果或即便认同原有结果但仍坚持重复检查。结论三级医院医疗检查结果互认实施效果一般,仍需通过加快构建三级医院医疗信息服务平台、促进医疗资源共享等方式推进三级医院医疗检查结果互认实施工作。Objective To analyze the status of mutual recognition of medical examination results in tertiary hospitals and its influencing factors,and provide scientific basis for government decision-making.Methods 342 patients in the pilot medical institutions from June to August,2018 of tertiary hospitals were examined,and the data were analyzed by stepwise regression analysis.Results Only 59.19%of the patients had a hearing on the mutual recognition of the tertiary hospitals.30.53%of the patients were unclear about the mutual recognition of the tertiary hospitals,and only 5.61%of the patients were very familiar with the mutual recognition of medical examinations in tertiary hospitals.Patients with different medical expenses had different attitudes toward repeated examinations,and the difference was statistically significant(P=0.000).The doctors still maintained a conservative attitude to the use of the patients'original condition examination data,and more than 50%of patients said that in the process of medical treatment,the tertiary hospitals didn't use the original medical examination results or even approved the original results,but still insisted on repeated inspections.Conclusion The results of mutual recognition of medical examination results in tertiary hospitals are generally effective.It is still necessary to promote the implementation of mutual recognition of medical examinations in tertiary hospitals by accelerating the construction of medical information service platforms for tertiary hospitals,and promoting the sharing of medical resources.
